Position Title: Restorative Nurse Aide

Occupational Exposure: Category I

Department: Nursing

 

Reports To: Charge Nurse/Supervisor/Nursing Administrator

 

Position Summary: To coordinate, implement, and provide restorative nursing care for all customers in the center with the direction of the Nursing department and in conjunction with Rehabilitation, to maximize and maintain the physical functioning and emotional wellness of all the customers.

 

Responsibilities/Accountabilities:

1.         Oversees, directs and participates in restorative programs initiated for individual customers;

2.         Communicates on a frequent and ongoing basis with the Nursing and Rehabilitation departments concerning goal setting and approaches to individual customer’s needs;

3.         Assumes direct responsibilities for facilitating the transition of each customer from skilled rehab services to the restorative progressive and preventative programs;

4.         Attends rehab rounds and communicates to appropriate staff the status of all customers with regard to rehab/restorative needs;

5.         Maintains restorative nursing progress/documentation records on all customers participating in any restorative program;

6.         Visits each customer daily and/or as needed to assure that the program is appropriate and productive;

7.         Trains primary care givers in restorative techniques and assists in providing that care;

8.         Monitors all customers with ROM and T&P orders to insure compliance of the order;

9.         Instructs and assists primary care givers with the application of splints, braces, prosthetic appliances, and self-help devices to assure proper application and usage;

10.     Directs and participates in all programs under the restorative umbrella;

11.     Visits all new admissions to identify restorative needs and communicates those needs to appropriate personnel;

12.     Assists in communicating with families concerning customers progress in care planning conferences or on a one to one basis with the family and the customer;

13.     Spends time on all shifts as necessary to instruct and oversee restorative programs to insure continuity of care;

14.     Assumes responsibility to alert Nursing and/or Rehab of customers who may be experiencing a change in their level of functioning, which should be addressed;

15.     Remains familiar and proficient with using wheelchairs, walkers, canes, lifts, scales and other equipment used in the restorative programs and is able to instruct others in their use;

16.     Puts Customer Service First: Ensures that customers and families receive the highest quality of service in a caring and compassionate atmosphere which recognizes the individuals’ needs and rights;

17.     Performs other duties as requested;
