Position Summary: To provide individualized in home and community services and supports to attain a stable, supportive home environment, community integrated life style and educational achievement based on values of individual self-worth, dignity, equality and respect for maximum participation in routine everyday activities, experiences and challenges by implementing functional skills training, opportunities for social/interpersonal experiences, therapies, health services/supports, community support activities and supports for community inclusion to individual with a disability who have been in and are in jeopardy of placement in an institutional setting in order to assure maximum normalized living, independence and self-reliance while growing up and as an adult.
To improve or assure consistency in attained functional skills and provide structured community, social and interpersonal experiences/activities for normalized developmental growth and family stabilization while meeting IAG’s policy and procedure requirements and the guidelines of funding, monitoring and accrediting agencies.
¨ Follows schedule for assigned individual for hours of work for each designated individual on a weekly basis as prescribed by weekly schedule and in compliance with IAG policy and procedures for hours worked per work week and in accord with each designated individual’s individual service plan.
¨ Delivers of assigned work tasks in compliance with schedule of delivery relative to planned work daily start and end times in order to meet performance standards, meet expectations of individual and family for timeliness and meet objectives for use of resources.
¨ Implements services and supports following prescribed methods and techniques in accord with each individual’s specific service plan, discussing problems with implementation of methods and techniques with RSD, in order to assure appropriate delivery of services and supports in accord with individual service plans, IAG procedures and state rules.
¨ Determines and/or interprets work procedures consistent with plan and environmental context or situation in order to implement individual’s program of services.
¨ Communicates with diagnostic professionals, counselors/therapists, DCFS caseworkers, CASA court staff or probation officer on behalf of in order to provide accurate and updated interchange of information, intervention methods and recording of data.
¨ Completes reports, data sheets and other required information (tracking sheets, goals, progress notes etc.) following prescribed procedures and timeframes in order to meet record keeping requirements, evaluate services and to assure compliance with IAG and State standards.
¨ Regularly communications with RSD, assigned individual and families using pager, telephone or individual discussions, using discretion as to subject matter and time of communication, in order to coordinate service delivery, notify of changes in schedules or time of service, time of meetings, etc. for assigned individual.
¨ Participates in in-services using necessary materials in accordance with IAG policies and procedures in order to assure implementation of appropriate techniques for service intervention, compliance with planned program of services and to meet State rules to achieve greater professionalism in capability for implementation of services and to assure staff has been presented with IAG policies and procedures.
¨ Participates in meetings of an informational or problem solving nature, e.g., service planning meeting, parent meeting, diagnostic, intake/discharge and court contributing appropriate input, in order to establish and maintain effective communication, to assure professional program planning and monitoring and to perform necessary organizational business.
¨ Writes/develops sequence of steps or procedures by which client may achieve specified result or objective, adapting and modifying plan as changes in situations and/or client require, in order to implement effective interventions/supports and to monitor progress.
¨ Writes/reviews/evaluates individual’s program plan as assigned, referring to recommendations of team, client and client family, IAG staff, previous performance and outcomes, makes adjustment and changes using discretion as to need in order to formulate individual program plan.
¨ Writes incident reports using staff and client information, case records and knowledge of the situation, recommending plans for resolution in order to compile required records and information for service planning, assure communication of crucial events within IAG and to meet IAG procedures requirements and state regulations.
¨ Implements Wrap services and supports program following prescribed methods and techniques from the plan based on scheduled time for service delivery in accord with IAG procedures and state requirements.
¨ Reinforces proper behavior, language and social pattern etc., in order to improve or increase consistency of the independent functioning of the client.
¨ Exchanges with, receives from or provides to diagnostic staff, colleagues or supervisor information about client or client program in verbal or written format as required in order to maintain records, plan or implement services.
¨ Formulates client objectives and evaluates client’s performance on assigned objectives examining outcome against stated criteria, using rating scales, tracking sheets, monthly progress notes, etc. in order to ensure personal growth and improvement in specific skills.
¨ Inspects/monitors clients for appropriateness noting clothing, hygiene, behavior etc., in reference to standards and taking corrective action if required in order to maintain the dignity, health, welfare and safety of clients.
¨ Substitutes for absent coworker in performance of assigned duties with direction from supervisor in order to endure the day-to-day functioning of the unit.
¨ Completes required reports, forms, progress notes, and staff work schedules etc., using performance records, tracking sheets, and established guidelines in order to maintain compliance with agency and state requirements.
¨ Discusses in individual’s conference meeting, individual’s understanding of situations, behaviors, personal goals, attitude toward others and emotional consequences in order to inform other team members of ongoing behaviors and observations to evaluate current plan and develop services.
¨ ¨ Minimum of one year of directly related work experience or high school and five years of directly related work experience.
