Hospice Clinical Liaison (LVN)

The ideal candidate would provides information about hospice services to potential patients and families and obtain consents when appropriate. Meet and exceed the census goals by proactively interacting with the professional community to attain referrals and admissions. 

  • Communicates with staff, physicians, referral sources and prospective patients/families regarding hospice services.
  • Completes all consents and legal documents accurately and concisely.
  • Documents professionally, accurately, timely and according to established standards.
  • Develops new referrals from healthcare professionals, special interest groups, discharge planners, case managers and professional and community-based referral sources.
  • Collaborate with provider partners, community and state agencies, hospitals, SNFs and other partnerships providing education within targeted communities/populations.
  • Develops presentation materials for referral sources as necessary
  • Effectively and appropriately identifies and eliminates barriers to referrals and admissions
  • Understands and adheres to organizational compliance and privacy programs.
  • Assists in providing service that fosters the organization’s mission, values, vision and goals.
  • Perform other duties as required and/or requested by management.

  Hospice Clinical Liaison (LVN)

    • Three to five years experience in healthcare marketing or nursing degree preferred.
    • Hospice experience required.
    • Effective interpersonal skills.
    • Excellent communication skills, both oral and written as well as presentation skills.
    • Ability to function as part of a multi disciplinary team
    • Ability to travel to multi work locations during workday and evening as needed
    • Ability to work a flexible schedule
    • Ability to work with information systems/computers and input necessary information, including Word, Allscripts, Excel, etc
    • Must have reliable insured vehicle and valid Texas driver’s license.

The First Hospice in Houston

Houston Hospice was founded in 1980. In 1989, The Texas Medical Center acknowledged the importance of Houston Hospice in the health care spectrum by donating to the hospice the former 2.5 acre estate of Houston Mayor Oscar Fitzallen Holcombe. Thanks to community support, Houston Hospice opened the Margaret Cullen Marshall Patient Care Center in 1996. Also in 1996, Houston Hospice developed the Butterfly Program to provide for the distinct care needs of youth aged 18 and younger.

Houston Hospice is a nonprofit 501 (c) 3 organization that is community-supported. Our mission is to provide compassionate physical, social and spiritual support to individuals with a life-limiting diagnosis, and their loved ones, irrespective of their ethnicity or beliefs.

We believe no one should live in pain or in fear of being alone. Houston Hospice utilizes a team-oriented approach to medical care, pain management, and emotional and spiritual support tailored to each unique patient.

Local Houston Hospice teams provide care in patients' homes, in assisted living communities, and skilled nursing communities throughout ten counties. Teams include doctors, nurses, hospice aides, social workers, chaplains, bereavement counsellors, and trained volunteers.

The care team helps manage pain and symptoms, monitor the patient's condition, maintain hygiene, anticipate needs, assist with medical/insurance forms, listen with compassion, and give caregivers rest. This support helps caregivers have more energy and quality time with their loved one.

Houston Hospice is an in-network provider for most insurance companies and is licensed by Medicare and Medicaid. Houston Hospice is a member of the Texas Medical Center, Texas Nonprofit Hospice Alliance and the Texas New Mexico Hospice Organization. Houston Hospice is accredited by Community Health Accreditation Program, Inc.
