The primary responsibility of this position is to provide behavioral support to assist patients in improving health status and manage chronic illnesses. This position integrates the Behavior Health Consultant role with the primary care team by providing services to patients who could benefit from behavioral interventions. In addition, the responsibilities include screening/evaluation of individuals for mental health and substance abuse disorders; conducting brief interventions for individuals with behavioral health problems; meeting regularly with the primary care teams to plan care and discuss cases and exchange appropriate information with team members as part of the daily routine of the clinic.
Clinician must have demonstrated success in the implementation of symptom resolution and solution-focused approaches to client’s problems as well as the retention of clients. To perform this job successfully, an individual must be able perform each essential duty satisfactorily. Must have a valid Missouri clinical license in social work.
Must have a Master’s Degree in Social Work.
