1. RN License & Registration-New Jersey
2. copy of the front and back portion of your American Heart Association Accredited BLS/CPR card, ACLS and other certifications that you may have
3. 2 government issued IDs
4. 2 written letters of references
5. Latest physical – if you have an existing one, please ensure that it is less than 1 year old and should have your MMR Varicella titers plus lab reports, your PPD and/or chest x-ray (if positive) if not, you can use the attached forms.
6. copy of nursing diploma
