Our Locations and Service Areas
We proudly serve patients and families in private homes, assisted living facilities, skilled nursing facilities, hospitals, and our care centers located in Wilmington, Bolivia, and Whiteville, North Carolina. Our service area includes Bladen, Brunswick, Columbus, New Hanover, Onslow and Pender Counties in North Carolina; and Horry, Georgetown and Marion counties in South Carolina. Our corporate office is in the coastal community of Wilmington, NC.

We are currently seeking a Clinical Nurse Liaison to join our group of dedicated and experienced professionals.
This is an outstanding opportunity to join the largest and most experienced hospice in the area. Lower Cape Fear Hospice is certified by Medicare and Medicaid and approved by the Accreditation Commission for Health Care, Inc. Each day, more than 600 patients and their families receive care from Lower Cape Fear Hospice in our hospice and palliative programs.
Knowledge, Skills and Abilities:
Acts as a resource to physicians, patients/families, and assigned facility staff regarding end-of-life issues and palliative care.
Acts as a referral source to physicians and other medical staff in the hospital, long term care and facility settings to assess patients for appropriateness of admission to hospice/palliative services.
Coordinates transition of hospital inpatients to hospice/palliative home care, facility, or hospice inpatient facility admissions.
Coordinates transition of facility patients to hospice/palliative care.
Acts a liaison between LCFH and referral sources by providing service information to physicians within hospital, long term care and assisted living settings as well as patients and families and other providers of health care.
Works in conjunction with Palliative Care Nurse Practitioner and Physician.
Organizes and performs work effectively and efficiently.
Maintains and adjusts schedule to enhance program performance.
Demonstrates positive interpersonal relations in dealing with all members of the team.
Maintains and promotes customer satisfaction.
Effectively demonstrates the mission, vision, and values of Lower Cape Fear Hospice daily.
In facility setting, works closely with QI Department to conduct chart reviews of hospice patients in facilities.
Graduate of an accredited school of nursing, either through an AD, Diploma or BSN program.
Current license to practice professional nursing in the State of North Carolina.
Two years’ nursing experience required.
Hospice or home care experience preferred.
Prefer experience in marketing, public affairs, community relations or hospital liaison nurse.
Degree in business, marketing or related field preferred.
