Job Title: Full-Time Physical Therapist

Position Information: The Physical Therapist position will be responsible for managing a primarily orthopaedic outpatient population, with patients’ ages ranging from adolescent to active geriatric.  The clinic maintains a very reasonable caseload with each therapist seeing one to two patients per hour. There is excellent resources, up-to-date equipment, and support available. Clinic emphasis is on quality one on one care. Experience working in outpatient orthopaedics is a plus for this position, but not necessary.

Job Duties for this Role:

  • Plan, prepare and carry out individually designed programs of physical therapy treatment to maintain, improve, or restore physical functioning, alleviate pain, and prevent physical dysfunction in patients.
  • Perform and document an initial evaluation, using the data to identify problems and determine a diagnosis.
  • Evaluate the effects of treatment at various stages and adjust treatment to achieve maximum benefit.
  • Administer a treatment plan with re-evaluations throughout to modify the treatment plan to progress patients towards functional goals.
  • Instruct patients and family members in the treatment procedures that will need to be continued at home.
  • Confer with the patient, the medical practitioners, and other appropriate individuals involved in the patient’s care to plan; implement and assess the intervention plan.
  • Review physician’s referral/prescription and patient’s medical records to help determine the diagnosis and physical therapy treatment required.
  • Record the patient’s prognosis, treatment, response to treatment, and progress towards functional goals in the EMR system. 
  • Obtain the patient’s informed consent to proposed interventions.
  • Discharge patients’ when goals or projected outcomes have been attained or when therapy is no longer progressing towards functional goals.  
  • Be able to provide for appropriate follow-up care or referrals.
  • Test and measure the strength, sensory perception, motor control, and function of the patient’s; record data as appropriate.
  • Identify and document goals, anticipated progress, and plans for re-evaluation.
  • Inform the patient when evaluation reveals findings outside of the scope of physical therapy practice and refer the patient to an appropriate practitioner.
  • Direct and supervise appropriate supporting personnel, delegating specific tasks appropriately to them, and establish and maintain open and proper lines of communication.
  • Uses resources of the clinic (equipment, space, support personnel) appropriately, and recognizes when resources may need to be re-allocated to serve client needs and clinic workflow.
  • Administer treatment involving the application of physical agents.
  • Complete all training as is necessary to maintain compliance and working knowledge of EMR system to ensure production of appropriate documentation.
  • Finishes documentation in a complete and timely manner.

PHYSICAL THERAPIST

REPORTS TO: Physical Therapy Team Leader and/or The Office/Operations Manager

SUMMARY: This position is responsible for managing a primarily Orthopedic outpatient population, with patients’ ages ranging from adolescent to active geriatric; will also act as a liaison between patient and other members of the patient’s care team.

JOB REQUIREMENTS:

·  A degree in Physical Therapy from an APTA Accredited University

·  Must be licensed in the State of Illinois or eligible for licensure in the State of Illinois

·  Solid Work History

·  Good Work Ethic

·  Must have or be eligible for NPI number

JOB RELATED SKILLS/QUALIFICATIONS:

·  Having a complete understanding of orthopedic injuries, confidant evaluation skills, and a complete understanding of orthopedic protocols for acute, chronic and post-surgical rehabilitation.

·  Have a thorough understanding of the job tasks that are performed within a company and be able to recognized safety issues.

·   Develop a line of communication when dealing with an employee incident (i.e., workplace accident) including union stewards, worker compensation representatives, safety managers and the treating medical professional.

·  Become familiar and knowledgeable in regards to the company’s established safety guidelines in additional to familiarization of OSHA guidelines, OSHA reporting, and any corporate injury reporting procedures.
