Our Client is seeking a Case Manager in Camden, NJ 08103
Earn $17.00 per hour at this full-time, temp-to-hire position. Work 40 hours per week, Monday through Friday from 8:00 AM to 5:00 PM.
Job Summary:
The Case Manager is responsible for outreach services to TANF recipients receiving TANF benefits. This role is responsible for evaluation and determination of eligible individuals, dissemination of information outlining career planning, job coaching, employment and occupational training services available and referring to the Board of Social Services to begin the appropriate process for accessing services. In addition, they will provide follow-up to individuals who have been referred for services or expressed interest in pursuing any of the above noted career/job training services.
Job Description:
- Identify eligible individuals through information provided by Board of Social Service and its case workers
- Identify eligible individuals through information provided by Emergency Service programs and their case workers
- Conduct an initial meeting, intake and assessment of eligibility and needs, typically away from the program office
- Screen for potential mental health or substance abuse issues and arrange appropriate follow up
- Develop a plan with the person to become re-involved with work activities
- Explore or link to other services for which they are eligible and in need
- Make appointments for benefit recipients with their BSS Case Manager and assist them in making the appointment in any way necessary
- Confirm with the BSS case management staff that the client kept his/her appointment and re-establish the meeting, with measures to ensure success, if the appointment with BSS is missed
- Reassess obstacles and determine response to support participation, whether encouragement, concrete assistance such as transportation, linkage for services or advocacy
- Implement rewards and recognition ritual and materials
- Maintain contact with the BSS Case Manager to address problems with the plan of service and with the Career Coach to support development of a motivating career plan
- Complete all paperwork and reports in a timely manner as required by supervisors and funding sources
- Participate in assigned meetings and trainings as related to program
- Participate in the implementation and ongoing development of the program
- Ensure immediate and thorough communication to supervisor of program problems and provide recommended resolution to identified problem
- Maintain a positive work atmosphere by communicating in a manner that fosters good relationships with customers, clients, co-workers, external colleagues and management
- Perform other duties as assigned
Requirements:
- High School Degree or Equivalent (GED) plus 5 years of related work experience, or
- Bachelor's Degree in Social Work, or
- Associate's Degree in Social Work and 2 years related work experience
- CSW certification is preferred
- Bilingual - Speak fluent English/Spanish is preferred
- Excellent communication skills
- Must be able to write clearly, grammatically and concisely
- Knowledge of entitlement programs and program regulations, federal, state and community resources and expertise in navigating these resources
- Commitment to the mission of our company
- Ability to relate directly but compassionately with people in continuing life difficulty
- Ability to engage and gain rapport with “reluctant” clients with patience, directness and empathy
- Exercise discretion with the ability to assess client and family resources, strengths, goals and obstacles to movement to self sufficiency in a short period of time
