Position Summary:  To provide nursing care, health support services staff training for implementation of health services to people residing at IAG according to individual services plans, approved medical standards and guidelines established by the Departments of Public Health and Public Aid and other licensing or regulatory bodies.

 

To implement medical services, health support, continuity in nursing services and meet documentation requirements in order to meet individuals’ needs and state health regulations.

 

Duties:

 

  • Reads/checks calendar schedule for individual medical/health/dental appointments assuring transportation, staff accompaniment and cross checking with assigned QIDP, writing in both calendar and in individual chart disposition, and arranging for future appointments for services as needed in accordance with individual need and agency procedures in order to ensure health, safety and welfare of individuals.
  • Administers PRN medications as needed at individual location following written physician's order, verbal physician's order with follow-up written order and in accordance with individual's IPP, writing in individual's chart when administered and following agency documentation procedures in order to meet emergency medical incidents and to meet agency, state and federal guidelines.

 

 

 

  • Trains direct service staff and or QIDPs in delivery of health support services in accordance with individuals IPP, best practices procedures and individual specific effective methods/techniques in order to provide continuity in nursing services and to meet the health, safety and welfare of individuals.
  • Delivers health support services to individuals in accordance with an individual's program plan and prescribed methods and techniques following professional best practices principles in order to meet an individuals needs and to meet agency, state and federal guidelines.
  • Administers medications following written physicians' order, initialing on medication sheet as administered in accordance with agency documentation procedures in order to provide continuity of nursing services and to meet agency, state and federal health guidelines.
  • Reads/checks daily appointment calendar for due dates of individual's annual physicals, informing individual's QIDP, individual, appropriate staff and related programs (e.g., DT school) of appointment commitment in accordance with daily schedule and census procedures in order to meet agency, state and federal guidelines.
  • Schedules and confirms non-routine laboratory work and tests as ordered in writing or verbally by physician, writing date and time in appointment calendar and laboratory work/test in individual’s chart, in order to provide necessary health care to individuals and to meet agency, state and federal guidelines.
  • Assists physician as needed with annual or other periodic/planned physical examinations using necessary equipment and transcribing physician's orders, recommendations and comments in order to maintain individual’s health and to meet agency, state and federal guidelines.
  • Reads/reviews laboratory and test results reporting in writing or verbally to physician unusual or unexpected results in order to provide timely health care to individuals and to meet agency, state and federal guidelines.
  • Reads/reviews medication orders checking accuracy against physician's orders, reporting variances in writing to physician and or pharmacy in accordance with agency procedures and in order to maintain safe and accurate health services and to meet agency, state and federal guidelines.
  • Reads/reviews physician's order sheets checking for accuracy and content in preparation for physician's signature in order to ensure accurate documentation, and to meet agency, state and federal guidelines.

 

 

 

 

 

 

  • Reads/reviews new medication sheet for time period checking accuracy of listed medications against physician's order in accordance with agency documentation procedure and in order to meet agency, state and federal guidelines. Reads/reviews current medication sheet checking for nurse's initials against medication administration and medication changes against physician's order in accordance with agency's documentation procedures and to meet agency, state and federal guidelines.
  • Assists physician with individual’s examinations using necessary equipment and transcribing physician's order, recommendations and comments in order to maintain individual’s health and to meet agency, state and federal guidelines.
  • Transcribes physician's orders for medication or treatment changes and reevaluation on medication sheet in accordance with agency procedures and to meet agency, state and federal guidelines.
  • Contacts pharmacy-using telephone placing orders or reorders (as needed or prescribed) for prescriptions following physician's orders on medication sheet in order to meet individuals' health and medication requirements.
  • Administers medication orally, intramuscularly or as suppository at scheduled times following physician's written order in order to meet individual's health and medication requirements.
  • Signs and initials mediation record form following administrator of medication to an individual keeping an accurate record of medication delivery in order to meet agency, state and federal guidelines.
  • Checks/monitors all individuals for changes in physical or mental status comparing current status against reference status recording in writing changes in individual’s chart in order to maintain timely and efficient health care.
  • Contacts physician, QIDP using telephone or personal contact any and all acute changes in individual’s physical or mental status in accordance with agency procedures in order to provide timely and efficient health care and to meet agency, state and federal guidelines.
  • Writes in individual's chart following periodic schedule documenting physical and mental status in PRN, treatment and response/results, response to medications etc in order to provide continuity in nursing services and to meet agency, state and federal guidelines.
  • Writes in chart of individuals who are exhibiting current medical problems indicating physical/mental status in accordance with documentation procedures in order to provide.

 

  • Continuity in nursing services and to meet agency, state and federal guidelines.
  • Administers treatments and procedures following physician's order documenting treatment response in periodic notes in accordance with agency nursing and documentation procedures in order to meet agency, state and federal guidelines.
  • Writes documentation of incidents resulting in injuries to individuals, staff or visitors recording and filling out appropriate forms, individual’s charts, nursing notes and injury report form in accordance with agency procedure and in order to meet agency, state and federal guidelines.
  • Observes and physically checks or calls on all individuals identified as being discharged from ER or Hospital within 24 hours delivering nursing services following physician's order, writing observations in nursing notes and notifying physician in event of acute change in status in accordance with agency procedures in order to provide continuity in nursing services and to meet agency, state and federal guidelines.
  • Performs miscellaneous duties as assigned by supervisor in order to ensure the day-to-day functioning of the unit.

Education/Experience:

  • Graduate of accredited nursing program
  • Current Illinois / District of Columbia Nursing license.
  • Certified by DDS/DHS as a Nurse Trainer
  • Minimum of 2 years experience, preferably with DD population or mental health.
