RN Case Manager and/or Clinical Supervisor - Home Health
Currently we are looking for one full -time Clinical Supervisor and one Case Manager. As a full-time Clinical Supervisor at Oxford HealthCare you will perform periodic visits to ensure continuous provision of superior care. This position plays a tremendous role in providing leadership to a team of talented clinicians. Experience with adults, wound care, diabetic management, Accu-Chek, and blood pressure management is a plus. All charting is done electronically.
Already working full-time elsewhere? NO PROBLEM, part-time/per diem opportunities for our case manager positions are available.
Responsibilities and Duties
Accurate completion of time sensitive documentation
Ensure implementation of appropriate plans of care
Establish accuracy of patient records in compliance of state and federal regulations
Conveying community resources for client and family education
Medi-Cal and Medicare experience
Highly active in quality assurance participation
Provide developmental feedback and
Execute training requirements as planned by Director of Nursing

Dedicated support team
Current RN license in the state of California
Two plus (2+) years of paid clinical experience as a RN in home health experience
Have valid driver's license and reliable transportation to conduct case visits
Spanish or Bilingual speaker is a plus, but not required
Prior use of Electronic Medical Records (EMR) preferred
