Provide skilled nursing Care to the sick in their homes, works in team relationship with the professional nursing staff, the LPN and others, and carries assigned responsibilities.Work under the supervision of the Supervisor of Clinical Services.
KNOWLEDGE, ABILITIES AND POSITIONS REQUIREMENTS
Comprehensive knowledge and skill in current nursing practice.
Knowledge of the concept of team and primary care nursing and the implementation of both models of care delivery.
Knowledge of the community resources.
Ability to make meaningful observations, evaluations and reports.
Ability to recognize the needs and concerns of patients, caregivers, and the multidisciplinary team to maintain therapeutic relationships.
Ability to accept and constructively utilize supervision.
Ability to provide skilled nursing care based on scientific principles, the nursing process, basic behavioral concepts, and intelligent professional judgment.
Graduation from an accredited school of nursing. Possession of a current Connecticut license to practice as a Registered Nurse.
Must perform other duties as required by the Supervisor.
Valid driver’s license.
Acceptance of variety of patient/caregiver lifestyles and ability to adapt to associated care delivery needs.
