JOB SUMMARY:  The Crisis Care Nurse (CCN) provides direct nursing interventions per the approved Plan of Care on patients in the Continuous Care level of care.  The Crisis Care Nurse observes and informs in a timely manner the client's designated team member of changes in patient's condition/response to interventions.  Changes in the plan of care and interventions implemented are documented immediately.  All care directly provided or coordinated by the Crisis Care Nurse is provided in conformance with federal/state regulations.   REPORTS TO:  Clinical Manager – Crisis Care   DIRECTLY SUPERVISES:  None   CERTIFICATIONS/LICENSES REQUIRED: Requires a valid, current Florida LPN or RN license and a valid Florida driver's license.   EXPERIENCE/SPECIFIC SKILLS AND ABILITIES REQUIRED: Minimum one year of clinical experience, preferably in hospice, home health, oncology, acute care or public health.  Ability to read, write and speak English.  Bi-lingual preferred.  Ability to manage time and triage phone calls received from patients, families and physicians.  Excellent communication and interpersonal skills.    PHYSICAL REQUIREMENTS:  Ability to lift, push, pull twenty five (25) pounds without difficulty.  Good visual and audio acuity.  Able to work under varying degrees of stress.  Ability to work in various facility and home environments including those with exposure to tobacco, smoke, and pets.   PRIMARY RESPONSIBILITIES: Direct provision of nursing services to patients in "Continuous Care” level of care based upon the plan of care. Collects and analyzes data from patient's medical history, nursing assessments, and staff input on an ongoing basis. Provide keen clinical observation of patient's condition, response to treatment/medications and recommend changes to patient's plan of care. Communicates changes in patient's condition to identified facility team members. Provide nursing visits as needed to facility patients as directed by the PCC-Crisis Care or designee.  Consistently documents observations and interventions in accordance with the hospice plan of care, all facility policies and procedures, Federal/State regulations and Medicare/Medicaid guidelines. Maintains strict confidentiality and respect for patient/family privacy and possessions. Practices Universal Precautions and safety at all times. Teaches patient/family the skills needed to maintain patient's comfort and perform personal hygiene activities. Provides emotional support to patients, family and other caregivers. Other necessary duties as assigned by supervisor.   SECONDARY RESPONSIBILITIES: Increases knowledge of palliative care through active participation in educational opportunities. Assists patients/families with understanding the Continuous Care level of care provisions. Attends facility General Staff Meetings, Clinical Services meetings and educational training sessions as scheduled by the Patient Care Coordinator-Crisis Care. Must agree to and sign facility Per Diem Agreement.

  • Suggest referrals to wound care consultant or wound care center
  • Accompany wound care team on weekly skin/wound rounds
  • Utilize acquired wound care skills
  • Preparing, wound care cases
  • Compile weekly wound statistics for monthly wound report
  • Develop comprehensive wound management plan and assist staff in wound care
  • Deliver patient care/treatment to accomplish wound care tasks
  • Provide comprehensive wound care management to patients
  • Perform wound care and other services
  • Register new wound care and hyperbaric patients
  • Ensure optimum patient care delivery in wound care nursing procedures
  • Participate in wound care rounding, wound care committee meetings and quality council
  • Reschedule wound care and hyperbaric appointments
  • Provide staff education related to wound care
  • Educate staff on wound care prevention
  • Assist physician with wound therapy
  • Conduct weekly wound care assessment/documentation updates
  • Serve as a primary wound care educator for patients / families with wound care issues
  • Ordering wound care supplies needed to provide wound care according to current best practice standards
  • Provide consistent quality wound care for home health patients
