POSITION TITLE: COMMUNITY NURSE LIAISON
Community Nurse Liaison has primary responsibility for planning, scheduling
and implementing community health, wellness and education programs and coordinates
preventive programs in conjunction with the local Directors of Health.
Community Nurse Liaison also performs as the Agency/Community advocate and is
responsible for the Agency’s Infection Prevention & Control Program, and
Emergency Preparedness Program.
Community Nurse Liaison reports directly to the Executive Director/CEO and trains
and supervises Community Health nursing staff when appropriate.

POSITION QUALIFICATIONS: COMMUNITY NURSE LIAISON
Registered Nurse or LPN with a current license to practice in the State of Connecticut.
Minimum of one-year full time nursing experience. Health and wellness promotion,
and community health education experience are desirable.
