Requirements:

  • One full year of experience within the last three years
  • Current FL Certified Nurses Assistant License
  • Current certification showing 75hrs of training (or Florida CNA certification)
  • Current CPR certification required
  • Physical within the last six months
  • Current PPD or Chest X-ray
  • Criminal history report with no activity listed
  • Pre-employment drug screen required
  • FL Alzheimer's CEU (must be 2 hours)
  • FL HIV/AIDs CEU
  • Adheres to the written and verbal instructions of the skilled nurse/therapist in providing patient care.
  • Provides assistance with hands on personal care, hygiene and activities of daily living as directed. 
  • Provides assistance with ambulation or exercises programs as an extension of therapy.
  • Observes for and reports any safety hazards found in the patient's home.
  • Observes for and reports any changes in the patient's condition or other significant observations regarding the patient.
  • Attends staff meetings and completes twelve (12) hours of in-services annually.
  • Reports any patient concerns/complaints or grievances to the Case Manager / Clincial Director.
  • Maintains patient confidentiality; complies with HIPAA requirements and Agency policies and procedures.
