Community Advocate (Case Manager)

CHOICES is focused on interrupting the cycle of domestic violence by responding to those in need, educating our community and advocating for social change.  Our state-of-the-art facility provides temporary shelter, counseling & support groups, legal & community advocacy and responds to a 24-hour crisis, information and referral hotline. 

We are in search of a full-time professional to be responsible for providing customized community-based support and services to adult survivors of Intimate Partner Violence/Domestic Violence (IPV/DV) who do not require shelter. Includes assessment of strengths and needs, development of  action-oriented goal plans and coordination of referral and linkage resources to assist clients in successfully and quickly addressing their challenges and needs; Using a trauma-informed approach as well as other community advocacy best practices with a primary focus on safety.


Essential Duties and Responsibilities:

Work with clients to develop action-oriented goal plans with a focus on safety and stabilization as well as resolving the unique challenges and needs of each client’s circumstances, operating from a trauma-informed and community advocacy approach.

  1. Implement planning practices that reflect client strengths, needs, responsibilities and action steps to help them achieve their goals;

  2. Monitor and evaluate progress throughout the goal attainment process, adjusting as needed;

Assist clients with referral and linkage to community-based social service and support systems that promote strengthening safety and stability.

  1. Refer clients to partner agencies and community resources that benefit their goals;

  2. Gather information from the larger community, creating working relationships and disseminating information;

Maintain clear and accurate case notes and data reports, carefully documenting all client contact and interactions


Provide various client groups services, as needed


Participate in all appropriate agency meetings, working with colleagues toward positive client outcomes.

Contribute to an atmosphere of safety, respect, dignity, support and resources through adherence to the CHOICES Code of Ethics and Confidentiality Agreements.

Perform other duties as required



  • Bachelor degree in social work or related field preferred or equivalent experience required.  LSW preferred;



  • Experience with Crisis Intervention, Case Management and Advocacy

  • Commitment and ability to work as a team with diverse staff, volunteers and clients; using a Trauma-informed Care, approach, preferred


Required Knowledge, Skills and Abilities

  • Knowledge of DV, community resources and ability to engage clients experiencing trauma

  • Sensitivity to the needs of agency clients and ability to maintain confidentiality

  • Strong oral and written skills

  • Knowledge of human service delivery systems and systems advocacy

  • Ability to work independently and as part of team to deliver services

  • Knowledge of basic computer skills


Other Requirements:

Valid Ohio driver’s license, access to reliable insured transportation and ability to meet LSS insurance standards required
