Position: Care Coordinator 
Office Location: Brooklyn 
Travel Boroughs: Brooklyn & Manhattan or Manhattan & Bronx
Hourly Rate: $20
*Office site is located in Brooklyn, Must be comfortable reporting to this location daily.


  • Responsible for individual caseload of 40-45 adult clients, each of which is to be visited one face-to-face and one telephone once a month.
  • Assure that clients attend medical appointments at outpatient clinics as well as comply with medication regimen.
  • Helps participants obtain benefits including food stamps, on-going financial and housing needs, health care and obtain access to transportation.
  • Assists participants in acquiring life skills: budgeting, housekeeping, nutrition, time & stress management, community integration, and interpersonal skills.
  • Research, evaluate and recommend community resources to meet non-medical needs of clients 
  • Initiate appropriate referrals to additional services including home care and other necessary referrals as needed.
  • Collaborate and meet with all members of the team, primary care and specialty doctors, inpatient providers and all partners on a monthly basis 
  • Facilitate and participate in client Case Conference for medical/psychiatric and peer review.
  • Acts as a linkage to community services including medical, behavioral, residential, entitlement and any other needed services
  • Provide verbal and written illness management educational tools and provides supportive counseling.  
  • Immediately reports serious incidents, serious incident allegations, incidents, or sensitive situations to supervisors. Completes incident reports 
  • Write clear Progress notes and maintain meticulous documentation of services and visits in patient’s electronic medical record, closes encounters within 48 hours.
  • Conduct initial and annual assessments to document strengths, needs, goals and resources.
  • Mutually develops and implement and update care plans with patients, care providers, significant others and community providers 
  • Provides crisis intervention and stabilization as needed.  May require some after hour’s follow-up to emergency situations.
  • Bachelor's Degree in Human Services or related degree
  • Minimum (2) years of experience providing direct services to mentally disabled, substance abuse and homeless
  • Bi-lingual- Spanish plus
