What are we looking for in a Case Manager - Chemical Dependency Professional?

Working with a multidisciplinary team, Case Manager CDPs provide supportive counseling, case management, and chemical dependency treatment to clients within designated outpatient programs. Using motivational interviewing and excellent communication skills, Case Manager CDPs will engage clients in treatment early and assist the client in achieving treatment goals. This position is best suited for someone who is organized, can build rapport quickly, and is knowledgeable about recovery principles. This role has a standard Monday - Friday day shift schedule and will work in our Sunnyside clinic location serving clients of all ages.

Case Manager CDP duties may include:

  • completing chemical dependency assessments
  • facilitating behavioral health treatment & education groups
  • collaborating with each client to create an individualized treatment plan
  • coordinating with Comprehensive treatment teams & community providers to expedite treatment for each client
  • advocating for clients to obtain services & benefits, as appropriate
  • completing all progress notes and documentation in a respectful, client-centered manner

Qualifications:

  • Bachelor's degree in social work, psychology, social services, or related field
  • Must have Chemical Dependency Professional (CDP) certification in Washington State, or have Chemical Dependency Professional Trainee certification and be within one year of obtaining full CDP certification
