Essential Functions Statement(s)
• Complete necessary intake paperwork in a place where the consumer is most comfortable.
• Provide individualized support and instruction to persons with disabilities placed and supported in community
integrated jobs or volunteer sites.
• Maintain detailed record of on-site performance by completing case notes, using the FAP (Fact/Analysis
• Maintain frequent contact with the consumer, families, SC's and any other team members.
• Attend team meetings and 1:1 meetings when scheduled.
• Assist consumers with travel related issues such as providing transportation and travel training.
• Work in conjunction with employers, to design individualized training plans, task lists and fading plans in
accordance with job requirements and the needs of the consumer.
• Track all work done for consumers using the SetWorks. Track work expenses on
Expense Report.
• Work effectively with your team members, members of the business development team and CIS employees in
specialized areas to support consumers in the best possible way.
• Obtain consumer's schedules and report all schedules and appointments to the scheduling team via the central
appointment and consumer schedule forms by the required due date. Report any changes to the schedule
• Analyze and develop support strategies that may be used to the consumer do their job tasks. Emphasis on those
most natural to the workplace.
• Gather detailed information on each task the consumer is to complete, the type of assistance needed, strategies on
fading and identify natural supports. This is to begin on the first day of work.
• Abide by all policies and procedures that are outlined in the Employee Handbook and Service Delivery Handbook.

Education: Bachelor's Degree (four year college or technical school): Preferred
Experience: 1 plus years of experience in education, social services or business
Computer Skills: Able to effectively use Microsoft Office products, specifically Outlook, Excel and Word.
Certifications &
Possess valid driver's license and automobile insurance.
Other Requirements: Must have access to an automobile and internet access at home. Able to pass all required
background checks. Fluent in American Sign Language if working with the Deaf and Hard of
Hearing team.

Since CIS employees work at many different locations and job types, the work environment is ever changing. The
employee may be subject to working outside in extreme temperatures and poor weather conditions. Employees may
also be required to work at facilities that house animals. Indoor environments may be noisy, contain smells, and
extreme temperature variations.
