Job Description

POSITION:  Home Health RN Case Manager

SUMMARY:

The RN Field Nurse is responsible for the timely performance, supervision, training, and documentation of clinical service delivery to the patient and his/her caregiver in accordance with accepted standards of nursing practice and regulatory guidelines.  S/he assists the RN Clinical Team Manager in improving the clinical and financial performance of the location.

 

Job Responsibilities include:

 

  1. Ongoing interdisciplinary assessment of the patient;

  2. Development and evaluation of the plan of care in partnership with the patient, representative (if any), and caregiver(s);

  3. Providing services that are ordered by the physician as indicated in the plan of care;

  4. Patient, caregiver, and family counseling;

  5. Patient and caregiver education;

  6. Preparing clinical notes;

  7. Communication with all physicians involved in the plan of care and other health care practitioners (as appropriate) related to the current plan of care;

  8. Participation in the Home Health Agency’s QAPI program; and

  9. Participation in Home Health Agency’s-sponsored in-service training

    KNOWLEDGE, SKILLS, ABILITIES AND OTHER CHARACTERISTICS:

     

     

  • Must be a Registered Nurse (RN) licensed in the state of practice or in accordance with the Board of Nurse Examiners rules for Nurse Licensure Compact (NLC).

  • Minimum of two (2) years of active experience as a RN, preferred.

  • Possession of a working knowledge of Medicare/Medicaid home health care benefits, policies and procedures preferred.

  • Understanding of Medical Diagnosis Coding, preferred

  • Has a positive attitude and willingness to learn throughout employment.

  • Self-starter with excellent verbal and written communication skills.

  • Willingness to learn and utilize software solution, inclusive of Divisional Work Flow Tasks.

  • Must be organized and have the ability to manage multiple tasks & priorities at any given time.
