The Care Manager is responsible for core services of health home care management including comprehensive care management, care coordination and health promotion, comprehensive transitional care, youth and family support and referral services while using technology to link services.
This position involves creating, documenting, executing and updating the individualized plan of care to identify and integrate all providers directly involved in the child’s care.
Position Focus Care Manager:
Support patient/family including utilizing peer support, support groups, and self-care programs
Increase patient’s knowledge about their disease, engagement and self-management capabilities
Knowledge of community/social support service to identify available community-based resources
Actively manage appropriate referrals, access engagement, follow-up and coordination of services
Use health information technology to record data accessible to multi-disciplinary team of providers
Relationship management: engage youth and families, community partners, and stakeholders
Handle crisis intervention in solution-focused manner
Will become certified CANS (Child and Adolescent Needs and Strengths)
Handles confidential information w/discretion according to agency policy and HIPAA regulations
Requirements Care Manager
Bachelor’s Degree in Social Work or related field, or
Bachelor Degree, plus NYS teacher’s certificate or NYS licensure/registration as a Registered Nurse, or
Bachelor’s Degree (any field) with 5 years of experience w/behavioral health diagnoses or CASAC
2 years’ experience with people w/ serious mental illness, developmental, or substance use disorders (or 1 year experience and have Master’s Degree in related field)
Working knowledge of MS Office and other computer related programs
Current, valid driver’s license per Agency guidelines; travel required
Ability to work a flexible schedule, including night and weekend hours as necessary
