D&S community services, a leader in the provision of long term therapy services, is currently seeking a fulltime Occupational therapist to provide home based services in the Jackson, TN area.   We are looking for motivated, passionate and self-directed applicants to join our exceptional therapy team as we grow our home based service across West Tennessee Region. Strong time management, organization and documentation skills are essential.   Traveling with incentive compensation within a 100-mile radius is required.   Current license in the state of TN required.


  1. Conducts initial diagnostic evaluations and screenings to assess the capabilities and limitations of the service recipient.
  2. Implements occupational therapy treatment activities and programs which are consistent with the physician’s orders and plan of care.
  3. Plans and implements appropriate treatment plan of care and training programs.
  4. Observes, records and reports on service recipient’s progress.
  5. Provides training on staff instruction protocols to direct care staff involved in teaching daily living skills and activities.
  6. Prepares and maintains operational records such as service recipient’s evaluations, re-evaluations, treatment plans, revised treatment plans, progress notes, discharge notes, and other operational records and reports, and submits in established format and timelines.
  7. Communicates with supervisor and other health team members regarding resident progress, problems and plans as needed.
  8. Attends consultations, staffings and medical staffings as appropriate
  9. Provides appropriate supervision of Certified Occupational Therapist Assistant.
  10. Complies with state regulations and national standards as they apply to the practice of Occupational Therapy.
  1.  Recommend Adaptive Equipment for mealtime, grooming, bathing, toileting and oral hygiene
  2. Evaluation & treatment for Sensory processing deficits/ defensiveness
  3.  Consults on Environmental control and accessibility needs
