Under the direction and supervision of the Director of Pharmacy:

Provides supervising medication reconciliation pharmacy services to all inpatients, outpatients, Emergency Services, employee and, nursing home residents,( if and when necessary).The Medication Safety Manager is a member of the Pharmacy Leadership team. Plans, organizes, directs and operates the Medication reconciliation team according to CVMC goals, policies and practices. Participates in the development and execution of the department strategic and operational plan as it relates to drug diversion surveillance, medication reconciliation, medication quality and safety.

Provides leadership and direction for all clinical and professional pharmacy staff and ensures that the medication needs of inpatients and outpatients are accurately met and maintains all necessary records. Develops systems of care, solves problems and facilitates processes required to meet customer need and achieve network objectives within budgetary targets. Oversees professional staff training and competencies. Develops a vision for professional development, growth and division. Additional responsibilities include performance management of assigned staff, quality measurement and process improvement, practice standards within area of expertise and regulatory compliance.


Bachelor of Science or Doctor of Pharmacy degree from an ACPE accredited school of pharmacy required. Doctor of Pharmacy degree preferred. Board Certification, fellowship or residency preferred.

Active, unrestricted pharmacist license in the State of Vermont is required.


Two years Hospital Pharmacy experience required. Demonstrated expertise in pharmaceutical care and drug distribution/control systems, medication reconciliation, quality improvement projects and medication safety preferred. Demonstrated ability to coordinate and facilitate work teams to achieve specific results.
