Qualifications

Minimum Job Requirements

  • LPN required, RN preferred.
  • Two years prior experience working as a nurse with individuals with intellectual disabilities and/or severe behavior challenges required.

Knowledge, Skills and Abilities Required

  • Knowledge of supportive care principles for the IDD population.
  • Ability to perform nursing assessments and patient triage.
  • Ability to educate individuals and/or families as to the nature of health issues and to provide instruction on proper care and treatment.
  • Critical thinking skills.
  • Ability to plan, implement and evaluate individual patient care programs.
  • Ability to maintain quality, safety, and/or infection control standards.
  • Ability to gather data, compile information, and prepare reports.
  • Basic knowledge of managed care.
  • Ability to read, understand, follow and enforce safety procedures.
  • Ability to work autonomously.
  • Ability to maintain emotional stability to cope with emergencies and other stressors.
  • Strong organizational and coordination skills.
  • Strong interpersonal skills.
  • Ability to prioritize and reprioritize quickly.
  • Knowledge of ethical principles and federal and state regulations.
  • Strong written and verbal communication skills. Ability to work effectively at all levels in a collaborative, customer service oriented, team environment.

Conditions of Employment

  • Appropriate current nursing license and CPR certification required.
  • Must maintain certification status.
  • Excellent physical assessment skills.
  • Current driver’s license.
  • Must be able to lift/transfer a minimum of 50 pounds.
  • Good interpersonal and communication skills

Overview

Under direct supervision of the Nurse Manager, provides skilled nursing care to the individuals on their caseload as well as comprehensive coordination of health care for those individuals.  Nurse Navigator will also serve as the first point of contact for families, agencies, and the interdisciplinary team.



Responsibilities

  • Provides direct nursing care for all individuals.
  • Performs nursing assessments and triage of each individual’s needs.
  • Has knowledge of and demonstrates computer proficiency.
  • Oversees the planning, scheduling, and implementation of health related appointments, including transportation to and from.
  • Routinely speaks with families and funding agencies regarding health related matters for all individuals.
  • Completes required paperwork in a timely fashion.
  • Follows up on medications and durable medical goods as needed to guarantee safe and efficient services and supports.
  • Works collaboratively with the interdisciplinary team to coordinate services for individuals. Interdisciplinary team includes nurses, medication trained staff, residential staff, physicians, clinical and care coordination staff, and members of the support services team(s) at Woods.
  • Provides education to individuals, families, and guardians and acts as an information resource to each individual’s health related matters.
  • Collects and reports data from each individual regarding barriers, health care, medications, prior treatments, etc.
  • Monitors and reports to appropriate team members the individual’s compliance with patient treatment plan.
  • Attends interdisciplinary team meetings.
  • May participate in the orientation, training and evaluation of new staff.
  • Follows established departmental policies, procedures, and objectives, continuous quality improvement objectives and safety, environmental, and/or infection control standards.
  • Collaborates with medical providers and patient care staff in the planning and implementation of patient and staff education.
  • Designs or participates in the creation of health education materials.
  • Performs miscellaneous job-related duties as assigned.
