Coordinates the interdisciplinary care of theirpatients. Ensures appropriate patient care and family involvement and educationand school/community consultation.

Responsibilities: A listingof the key responsibilities
  1. Supportspatient and families through procedures, medication therapy, emotional andfinancial issues, etc.
  2. Providestelephone triage, prescription needs, and notification and implication of labresults.
  3. Arrangesfor lab work and procedures. Provides timely notification to families of theresults of test procedures, radiology studies, blood work, and any other testsordered by the team.
  4. Assistswith patient and family needs and patient education materials.
  5. Documentspatient problems and progress.
  6. Participatesin interviewing and evaluation of staff.
  7. Workswith Nurse Manager to ensure adequate nursing coverage.
  8. Actsas multi-disciplinary liaison with staff from radiology, laboratory, financialservices, pharmacies, medical insurance companies, referring providers, and thein-patient units.
  9. Assistswith training and education of nursing students, medical students, and medicalresidents.
  10. Participates in relevant educationalofferings to enhance effectiveness and achieve required in-service training,and certification.
  11. Performs other duties as requested or assigned.

Minimum Qualifications:

  • Graduatefrom an accredited Nursing Program required.

  • Bachelorof Science Degree in Nursing (BSN) with 1 year of experience.

  • AssociateDegree (ADN) prepared nurses with 2 years' experience.

* AllAssociate Degree (ADN) prepared nurses must commit to completing a BSN orhigher nursing degree within 10 years of their hire date.

Required Licensure/Certification Skills:
  • LicensedRegistered Nurse in New Hampshire required.  
  • BasicLife Support (BLS) Certificate required.
