JOB SUMMARY:

A large national home care provider is seeking a full time Registered Nurse to administer skilled nursing care to patients in the home on an intermittent basis. In accordance with the physician’s orders, and while under the direction and supervision of the clinical supervisor, candidates may be required to perform OASIS and assess patients within regulatory requirements of OASIS and timelines.

JOB QUALIFICATIONS:               

• Graduate of an accredited Certificate, Diploma, Associate or Baccalaureate School of Nursing.
• Current State License as a Registered Nurse.
• 1 year of experience as a nurse in a clinical care setting or home health environment preferred.
• Reliable transportation required.
• Read, speak and write English in order to fulfill job functions in an understandable way.
• Sit, stand, bend, lift, and move intermittently. (Must be able to lift 80-100lbs).

JOB RESPONSIBILITIES:

• Assess assigned case load of home health care patients to identify the physical, psychosocial and environmental needs of patients.
• Make the initial evaluation and re-evaluate the patients nursing needs during each visit. Make follow up assessments according to OASIS and PPS regulatory guidelines.
• Responsible for communicating significant findings, problems, and changes in condition or environment to the clinical supervisor, the physician and/or other personnel involved with patient care.
• Participate in the implementation of the plan of care of an assigned case load to ensure quality, continuity of care, and achievement of patient outcomes.
• Revise the plan of care when necessary in response to identified patient care issues and notify the Clinical Supervisor.
• Follow appropriate preventative and rehabilitative nursing procedures.
• Document medication regimen and update medication profile according to established procedures.
• Counsel, instruct and include the patient and family in following the Plan of Care and meeting nursing and related needs.
• Follow all infection control guidelines and standards per Agency policy.
• Supervise and provide clinical direction to the Licensed Practical Nurse & the Home Health Aide to ensure quality and continuity of services provided.
• Communicate care coordination & change to the appropriate team members as they occur to the patient’s plan of care.
• Maintain current knowledge of, and promote compliance with, all federal, state and local regulations while remaining fully cognizant of current Medicare, Medicaid, and third-party reimbursement regulations and changes.
• Demonstrate effective time management skills by completing patient assignment within projected time frame and arriving to appointments on schedule.
• Show knowledge of current status of requirements of third party payer, by identifying and reporting potential payment/coverage problems to the appropriate clinical supervisor. (This includes but is not limited to Medicare, Blue Cross/Blue Shield, Medicaid and other 3rd party companies.)
• Responsible for protecting patient & staff privacy and confidentiality pursuant to HIPAA Privacy Final Rule.
• Provide positive, and supportive communication to physicians, patients, families, visitors, and other agency personnel.
• Performs other duties as assigned by the Supervisor.
