Summary: Facilitate and deliver all components of WellSpark Health’s Digital Diabetes Prevention Program for assigned classes. Deliver the Diabetes Prevention Program using a distance learning format; deliver the session content live to participants via WebEx interface. Perform additional responsibilities related to content creation, reporting, progress tracking; and provide one to one coaching to participants with a need and desire.


Requirements:

  • Responsible for preparing and delivering live engaging session content based on the prepared materials provided by WellSpark Health, to be delivered from DPP coach educators’ homes, delivering content and driving the WebEx, PowerPoint and other engagement activities simultaneously. 
  • Deliver required session content in an engaging and inviting way, responding to participants’ questions and other feedback throughout the course of the live session. 
  • Maintain oversight and understanding of how your classes are progressing in the program via available platform reporting, including attendance at sessions, collected physical activity data and overall weight loss progress.
  • Utilize motivational interviewing skills to ensure maximum participant engagement and provides coaching, information and support to empower participants to make ongoing independent healthy lifestyle choices.
  • Provide health coaching services to program participants outside of live session time, either telephonically, via WebEx video conference or through platform messaging.
  • Maintain control over the program workflow and structure; accurately document participant data into electronic chart in a timely fashion.
  • Invest in the health improvement of their participants and goes above and beyond to support them reaching overall program goals. 
  • Provide recommendations to enhance effectiveness of learning delivery.
  • Perform other duties as required.

Qualifications:

  • Bachelor’s Degree in health-related field (e.g., Health Education, Wellness, Health Promotion).
  • Health Coach Certification preferred.
  • Must achieve certification as a Lifestyle Coach in the National Diabetes Prevention Program. 
  • 3 - 5 years of coaching experience. (R)
  • Previous experience in diabetes education. (P)
  • Experience with providing engaging educational content and health coaching services across multiple modalities–telephonic, digital or a mixture. (R)
  • Must be able to effectively deliver content while responding to participants’ questions and other feedback throughout the course of a live session. (R)
  • Ability to effectively navigate and utilize multiple systems and points of technology independently. (R)
  • Must subscribe to high speed internet service and have reliable WiFi connection. (R) 
  • Bilingual capabilities. (P)
  • Background in motivational interviewing, active listening skills, Transtheoretical Model, mindfulness based, and other evidence-based coaching techniques. (P)
  • Experience in wellness related areas (weight management, nutrition, physical activity/health lifestyle behavior change). (R)
  • Experience in facilitating patient/client positive behavioral change. (R)
  • Ability to work hours likely skewed towards later afternoon and evening, time zone dependent.  (R)
