Registered Nurse Case Manager
Registered Nurse-Case Manager -Full-Time (Occasional Weekends)
General Summary
The Case Manger will facilitate the interdisciplinary plan of care of patients by focusing on the following:
Essential Functions
- Evaluate the appropriateness of clinic care, medical necessity, admission status, level of care, and resource management
- Coordinate activities that promote quality outcomes and patient throughput while supporting a balance of optimal care and appropriate resource utilization
- Identify potential barriers to patient throughputs and quality outcomes
- Facilitate appropriate discharge plans
Knowledge, Skills, and Abilities
- Registered Nurse with current license from the State of Illinois, BSN preferred
- Two years’ experience in an acute care setting
- One year of Certified Case Management experience, preferred
- Strong interpersonal communication and presentation skills
- Ability to make administrative/procedural decisions and judgements
- Ability to develop, implement, and modify multidisciplinary health care plans
- Knowledge of clinical and social services documentation procedures and standards
- Ability to supervise and train employees, including organizing, prioritizing, and scheduling work assignments
- Knowledge of community health services and social services support agencies and network
- Knowledge of billing procedures for clinical and social services
