Position Summary

As the Quality Assurance Nurse, you are responsible for the quality of care provided to hospice patients, families, and staff. You will monitor documentation, compiles regular reports, and works closely with the agency Director Clinical Services. S/he is responsible for applying the rules and regulations of state and federal regulatory agencies and other certified agencies.

PRINCIPAL ACCOUNTABILITIES

  • Participates in the development and update of patient care policies and emergency procedures.
  • Collaborates effectively with Executive Director, Director of Clinical Services and members of senior leadership team.
  • Provides consultation and education to colleagues to support professional development.
  • Communicates identified needs and potential solutions to supervisor.
  • Facilitates appropriate upward and downward communication.
  • Performs documentation reviews and audits to assess:
    • Quality of clinical documentation across all members of the IDT
    • Evidence of care coordination
    • Evidence of plan of care revision according to assessments
    • Compliance requirements are met
  • Shares identified documentation deficiencies and quality care concerns with Director of Clinical Services for colleague follow up planning.
  • Collaborates and coordinates efforts with Director of Clinical Services.
  • Manages and directs the Mentoring & Monitoring program and ensure compliance.
  • Reports deficiencies to appropriate staff for corrections and follows up to ensure completion.
  • Ensures that the plans of care are updated to reflect new orders, intervention, and instructions.
  • Assists program leadership with development and implementation of plan of correction following internal/ external survey/ audits.
  • Reviews IDT documentation for accuracy and completeness.
  • Reports deficiencies to appropriate staff for corrections and follows up to ensure completion.
  • Compiles quality data on a quarterly basis and submits summary to ED for Governing Body reporting
  • Demonstrates personal responsibility for professional development and training.
  • Participates in performance improvement programs.
  • Upholds a personal philosophy of integrity and commitment; communicates proactively when schedule changes are needed.
  • Models and facilitates communication between colleagues including medical records, business office, intake, clinical team, and leadership.
  • Provides input into program service recovery efforts.
  • Solicits and incorporates internal colleague feedback into quality efforts and performance improvement.
  • Serves as the administrator on call when scheduled.
  • Performs other duties as assigned.

Knowledge and Skills

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Education and/or Experience

Required: Registered Nurse licensed in the state of practice, 3 years of clinical experience.

Preferred: BSN strongly preferred but other degrees given consideration; experience in quality assessment, quality management, or data analytics a plus; experience with electronic medical records systems a plus.

Mathematical Skills

Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals.  Ability to compute rate, ratio, and percentage.

Language Skills

Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations.  Ability to write reports, business correspondence, and procedure manuals.  Ability to effectively present information and respond to questions from leaders, patients, colleagues, physicians, investors,  and the general public. Strong written and verbal communications.

Reasoning Ability

Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, or schedule form. Ability to interpret an extensive variety of technical instructions and deal with several abstract and concrete variables. Ability to think critically and perform continuous in-depth clinical/business process reviews.

Computer Skills

Proficiency in Microsoft Office suite is required, specific experience and comfort in Microsoft Excel is a plus.

Other Qualifications

Understanding and acceptance of hospice philosophy.  Must be of high integrity including maintenance of confidential information including all personnel and patient records. Ability to work a flexible schedule, to include some evenings and weekends. Must be able to travel. Strong organizational, interpersonal, and time management skills.

Certifications, Licenses, Registrations

Must be a Registered Nurse licensed in the state of employment. Must have a valid driver’s license and auto liability insurance.

Other Skills and Abilities

Ability to understand, read, write, and speak English.
