Supports Coordinator - Case Manager

CSS is working on a Temp-to-Hire opportunity for a respected, mission driven, Non-Profit agency in the Detroit Metro area. Our partner is need of an energetic and dedicated Supports Coordinator/Case Manager to coordinate supportive services to persons with intellectual or developmental disabilities. The selected individual will provide advocacy support in helping individuals learn, express, and join in society. THIS POSITION IS REMOTE FOR THE TIME BEING DUE TO COVID. THE SUPPORTS COORDINATOR WILL CONDUCT VIRTUAL FACE TO FACE (TELE-HEALTH) APPOINTMENTS WITH CLIENTS. ONCE TCOVID RESTRICTIONS ARE LIFTED THE ROLE WILL REVERT TO IN-PERSON OUTREACH TREATMENT.

 

Requirements:

  • minimum BSW Degree with minimum LLBSW licensure required.

  • Must have experience working with developmentally delayed individuals and their families.

  • Must have a valid driver's license and private, reliable transportation

  • Excellent Computer skills are required in order to complete reports via electronic record.

  • Must have excellent record keeping skills

  • Must be proficient in MS Office (Word, Excel, Outlook) and willing to learn additional systems as required
