Job Summary

The primary purpose of your job position is to direct, organize and develop all wound care services in accordance with applicable federal, state, regulatory and facility standards to ensure the delivery of quality resident care is rendered at all times, and that the Plan of Care and physician orders are followed with regard to wound care treatments. This position functions independently, with oversight by the Director of Nursing demonstrates above average communication skills, personal integrity and works effectively with residents, family members, physicians, staff and outside agencies as required. This position leads wound education, and supervises clinical staff in addition to providing direct wound care and treatments.  The Wound Care Nurse will deliver care competently to those residents with complex wounds, pressure areas, ostomies or continence needs. The Wound Care Nurse will assist nursing staff in identifying residents at risk of pressure ulcers; define early interventions for prevention and management.  This position reports directly to the DON.

Minimum Qualifications

Minimum Education: Graduate of an Approved RN program
Minimum Years of Experience: 2 Years of experience directly related to wound care
Substitutable Education & Experience: None
Required Certifications/Licensure: Must have a Certification in wound care and must possess a current, unencumbered license to practice as an RN in this state; Must possess a valid Healthcare Provider cardio-pulmonary resuscitation (CPR) certificate within 6 months of their entry into the role.

Essential Functions

  • Provides care and treatment for pressure areas, acute wounds, chronic wounds, diabetic ulcers, draining wounds, and fistulas.
  • Cooperates with physicians and other staff nurses in the development of treatment of wounds. Implements and carries out treatment as ordered by physicians.
  • Makes decisions concerning wound care in collaboration with the DON regarding treatment protocols when necessary. Notifies the resident’s attending physician of changes in wounds or sign of infection.
  • Initiates referrals when indicated for further medical or surgical intervention.
  • Requisitions and arrange for diagnostic and therapeutic services as ordered by the physician and in accordance with our established procedures.
  • Consults with resident’s physician and responsible party when there is a change in the resident’s condition.
  • Reviews the resident’s chart for specific treatments, medication orders, diets, etc. as necessary.
  • Maintain required records and reports.
  • Visualizes wounds/pressure injuries weekly, complete weekly Pressure Injury and Non-pressure Wound reports, and maintain reports in preparation for Wound Care meeting. Chairs the Wound Care meeting. 
  • Provides data to the Quality Assurance and Assessment Committee as requested.
  • Supervises, evaluates, and provides education to the treatment nurses and other nursing staff. Assist with orientation of new employees to specific job functions on the nursing unit and ensures that all nursing service personnel are knowledgeable of the residents’ rights and responsibilities.
  • Attends and participates in the Quality Improvement Committee, as well as other facility committees as needed/necessary.
  • Charts nurses’ notes in an informative and descriptive manner that reflects the care provided, as well as the resident’s response to the care.
  • Encourage attending physicians to review treatment plans, record and sign orders.
  • Transcribes physician’s orders to resident’s charts, treatment MARs, and treatment assessment records. Performs routine charting duties as required and in accordance with our established charting and documentation policies and procedures.
  • Reports all discrepancies noted concerning physician’s orders to the Lead Nurse/Admissions Nurse. Signs and dates all entries made in the resident’s medical record. Co-sign treatment nurse's documentation on the wound/pressure injury records where required.
  • Obtains weekly assessments and measurements of all pressure areas.
  • Assures that prescribed medication is administered to the correct resident.
  • Verifies the identity of the resident before administering the treatment.

Duties and Responsibilites

  • Orders prescribed treatment medications, supplies and equipment as necessary and in accordance with our established policies. Ensures that an adequate supply of stock, supplies and equipment is on hand to meet the needs of the residents.
  • Notifies the attending physician of automatic stop orders prior to the last treatment being administered.
  • Reviews treatment records for completeness of information, accuracy in the transcription of the physician’s order and adherence to stop order policies.
  • Completes corrective action and evaluations of Treatment Nurses.
  • Attends and participates in continuing educational programs designed to keep you abreast of changes in your profession, as well as to maintain your license on a current status.
  • Ensures that your assigned work/treatment areas are maintained in a clean and sanitary manner.
  • Ensures that treatment nurses participate in and conduct all fire safety and disaster preparedness drills in a safe and professional manner, follow established sanitation techniques and infection control procedures when isolation precautions become necessary, follow established departmental policies and procedures, including dress codes, and that only trained personnel operate nursing service equipment in a safe manner.
  • Participates in the development, implementation, and maintenance of the infection control program (for monitoring communicable and/or infectious diseases among the residents and personnel) and the procedures for reporting hazardous conditions or equipment.
  • Maintains a stock level of medication, treatment supplies, equipment, etc., to adequately meet the needs of the resident.
  • Monitors nursing procedures to ensure that nursing service supplies are used in an efficient manner to avoid waste.
  • Recommends specific wheelchairs, cushions and mattresses to present/treat skin breakdown.
  • Reviews care plans daily to ensure that appropriate care is being rendered and informs the Nurse Manager of any changes that need to be made on the care plan. Ensures that nurses’ notes reflect that the care plan is being followed, as well as reviews resident care plans for appropriate resident goals, problems, approaches, and revisions based on nursing needs.
  • Maintains the confidentiality of all resident care information.
  • Monitors nursing care to assure that all residents are treated fairly, and with kindness, dignity and respect.
  • Reviews complaints and grievances made by the residents and make a written/oral report to the Lead Nurse/Admissions Nurse indicate what action(s) were taken to resolve the complaint or grievance.
  • Assists in training department personnel in identifying tasks that involve potential exposure to blood/body fluids.
  • Assures that nursing personnel follow established procedures in the use and disposal of personal protective equipment.
  • Maintains a liaison with the residents, their families, support departments, etc., to adequately plan for the residents’ needs.
  • Performs all other duties as assigned.
