If you’re looking for a rewarding job helping people with disabilities become more independent and involved in their community, Benchmark Human Services is seeking a full-time Registered Nurse (RN) or Licensed Practical Nurse (LPN) for our group homes. We provide thorough training, great challenges, and even greater fulfillment as you help individuals reach their maximum potential.
Function as a liaison and advocate for the clients when dealing with physicians, psychiatrist, hospitals, and consultants including RD, OT,PT, SLP, etc.
Supervise health services for the department by providing staff training and direct intervention to meet the health care needs of the clients in the group homes.
Medication Administration Certifications to include Oral applications, topical and health related activities, G/J -Tube, and Insulin, along with consultation for consumer health and safety
Complete all documentation required for assigned individuals health and related care needs, as required by state and federal regulations
Participate in development of associated health risk plans to ensure appropriate supports for assigned individuals
Provide on-call services to designated group homes.
Other duties as assigned
Experience in long term care/community nursing preferred
Experience working with individuals with intellectual and/or developmental disabilities and/or mental illness
Valid State nursing license and Drivers License
Some traveling required
Must be willing to flex schedule according to the clients needs
