Work Hours/Shift

  • Full-Time Days, Part-Time Days also optional

  • 40 hours/week

  • Monday-Friday, roughly 8am-5pm but with flexibility due to nature of home health

You Will Be Responsible For:

  • Performs an evaluation, assessing function using a method which objectively measures activities of daily living such as, but not limited to, eating, swallowing, bathing, dressing, toileting, walking, climbing stairs, using assistive devices and mental and cognitive factors, documenting the measurement results in the clinical record.

  • Administers treatments as discussed in the plan of care involving application of physical agents, and manual techniques to improve and maintain function. Instructs, motivates, and assists the patient to perform various functional physical activities, activities in daily living, self- care activities, and exercises.

  • Measures and monitors effects of treatment at various stages and confers with occupational therapist to adjust treatments to achieve maximum benefit and outcome.

  • Assures that patient treatment outcome is directed towards development and restoration of functional skills and returning to or maintaining maximum potential.

  • Recommends and teaches the use of assistive devices and equipment to patients and families.

  • Instructs nurses, Home Health Aides, and family members to provide for continuous Occupational Therapy program.

  • Observes records and reports on patient progress, reaction to treatment and any changes in patients condition.

  • Reports critical changes directly to Care Manager and/or other members of the interdisciplinary team

  • Provides accurate and current documentation.

  • Participates in monthly case conferences.

  • Consistently meets productivity expectations.

  • Participates in Performance Improvement studies as requested.

  • Attends staff meetings, offers in-services as requested.

  • Maintains education to expand expertise and current knowledge.


What You Will Need:


  • Functions with a high degree of independence

  • Ability to delegate tasks to appropriate personnel as indicated by skill level and professional standing

  • Strong computer and technology skills


  • A working knowledge of community resources and an ability to refers patients and families appropriately

  • Home Care Regulations and Third-Party Reimbursement as it impacts care delivery


  • Minimum of six-months relevant occupational therapy experience


  • Recent, relevant experience in a Medicare-certified home health agency as a Occupational Therapist

  • Masters degree in Occupational Therapy


  • Current Occupational Therapy License in State of Practice

  • Valid Drivers License and current car insurance

  • CPR certified


  • Field supervision: Effectively delegates and supervises delegated care, e.g., care provided by the home health aide and the occupational therapy assistants.

Job Summary:

The Home Health Visiting Occupational Therapist (OT), under the general supervision of the Care Manager, provides occupational therapy to assigned patients and directs therapy assistants and Home Health Aides to provide quality care in accordance with physician orders under a plan of care. Assesses, plans, implements, evaluates total patient care and coordinates patient/family teaching. Assures compliance with therapy standards, federal, state, and local government regulations, and company clinical policies. Provides goal directed care leading to specific outcomes. May be on call for after-hours work. Coordinates and directs the home care patients care based on individual patient needs.
