Review admission orders for skilled criteria and/or rehabilitation orders. Conducts concurrent review to ensure criteria is met. When criteria are no longer met must inform Administrator and initiate discharge immediately. Interview resident within 24 working hours, and document in Medical Record. Confirm desired discharge plans. Provide insurance benefit coverage information. Ability to orient to plan of care and interdisciplinary team involvement. Determine the need for family conference and organize and facilitate. Attend and participate in weekly Utilization Review. Obtains timely insurance pre-certifications and provide weekly updates verbal/written for continued stay authorization. Coordinate and obtain authorization from insurance company for all procedures, appointments and transportation. Informs and counsels resident and family regarding alternative types of care and resources to be utilized while in facility and at time of discharge. Informs responsible party when resident is approaching the end of rehabilitation and nursing care status and interpret recommendations for continuation of care. Documents on a weekly basis in medical chart as required by department, regarding resident response to ELOS, status and discharge plans.
- Clear and Active Registered Nurse or Licensed Practical Nurse license to practice in the state of Florida, Social Work or related clinical field
- CPR certification
- 1-3 years of experience in a Health Care Facility
- Must have knowledge of computer office/clinical software
Must be able to read, write and understand the English language.
