Summary: Behavioral Health Counselor
Provides triage, assessment, and treatment to patients and families, assists with disposition planning and provision of education and resources initiates psychiatric holds
This LCSW-BH will be the Code Gray team leader prepared to facilitate the creation of a milieu for resolution and de-escalation for patients and their families that have some level of escalation in the Outpatient Clinics. The ambulatory LCSW-BH must have the experience and ability to handle high-level crisis situations, de-escalation skills, and work with an interdisciplinary team to facilitate transitions of care from the ambulatory space back into the community or the next level of care.Work Schedule: Full -Time Responsibilities:
Implements counseling/therapy for assigned patients in both individual and group settings dependent upon program-specific
regulatory requirements. Interviews and assesses patients and/or patient's families, caregivers, and/or legal representatives.
Determines provides and/or arranges for needed internal and external services/interventions. Participates and/or arranges
case reviews/care conferences to evaluate progress.
Assists with discharge planning and processes, including identification of community-based resources, and provides referrals
as part of the discharge plan. .
Utilizes appropriate teaching methods and/or referrals to ensure patient/family educational needs are met.Requirements:
Minimum Required Education: Master's Degree.
Required Licensure/Certification: State licensure as a Licensed Professional Counselor Candidate (LPC-C), Licensed Marriage
and Family Therapist Candidate (LMFT-C), or Licensed Social Worker (LSW).
Minimum Experience: None Required. Preferred: 2 years relevant experienceThe pay range for this position is $29.54 - $44.31/ hour. Pay is dependent on the applicant's relevant experience.
