Eckerd Connects' Wraparound Agencies are part of the Coordinated System of Care (CSoC) under the state's behavioral health managed care program that serves children and adults in Louisiana with behavioral health, mental health and addictive disorders. Coordinated Systems of Care are national evidence-based models for coordinating services in a family-driven manner to ensure more positive outcomes for children, youth and families. The model incorporates a broad array of flexible effective services and supports that is organized into a coordinated network and that integrates care planning and management across multiple levels and systems. Through this coordinated network, meaningful partnerships are built with families and youth at service delivery, management, and policy levels. Family-driven and youth-guided services are provided in the home as well as in community settings that are data-driven and outcome oriented. Treatment plans are individualized and strength-based, culturally and linguistically competent, and integrated across all child-serving systems.

Position Concept
The Assessment Coordinator is responsible for ensuring that youth and families referred to the agency are assisted through the authorization process with the Statewide Management Organization (SMO). For families that are deemed eligible for Coordinated System of Care services, the Assessment Coordinator will track the authorization process through the duration of client’s enrollment. The Assessment Coordinator will also refer non-eligible families to Directors for assistance with non-CSoC service options.

Essential Functions  

• Complete individualized assessments of each youth referred for Wraparound Process. 
• Ensures that services are provided in accordance with funding guidelines and are based on the specific needs of children, youth and families. 
• Ensures that families are offered choice of either institutional or home and community-based waiver services using the form LTC/CS (Long Term Care/Choice of Services). Ensures family participation and opportunities to identify appropriate goals, strengths, and needs in the development of risk assessments, crisis plans and plans of care. 
• Complete assessments, including a Child and Adolescent Needs and Strengths Survey (CANS), Independent Behavioral Health Assessment (IBHA) form, and other assessments as needed. 
• Serve as a liaison between the Statewide Management Organization, Wraparound Facilitators and families in assessments and re-assessments. 
• Supports Coordinated System of Care principles and values. 
• Ensures assessments are completed in a manner that is strength based, culturally and linguistically competent. 
• Ensures that Initial Assessments are completed and submitted to the Statewide Management Organization prior to the end of the 10-business day initial authorization period. 
• Assures that all Re-assessments are completed and submitted to SMO prior to the end of previous authorization ends. 
• Assist in establishing a cohesive multi-systemic alliance among community stakeholders to assure that staffing, billing and inter-agency processes are working effectively and efficiently. 
• Promotes services that are community based, evidence based and/or evidence informed. 
• Maintains confidentiality in all aspects of member care (i.e. HIPAA, FERPA, P.L. 46:56), staff and agency information.  

Other Duties Include, But Not Limited To  

• Must maintain CANS certification through SMO or state training. 
• Complete Louisiana CSoC Wraparound training and certification process as developed by the State Governance Board and Office of Behavioral Health, including all Child and Adolescent Needs and Strengths (CANS) training. 
• Maintain a high standard of role model behavior for youth and families. 
• Participate in training and development activities as required. 
• Report any acts, incidents or conditions that reflect the possibility of inappropriate youth-to-youth or staff-to-youth relationships. 
• Report any use of physical force and all unusual incidents per Eckerd and CSoC and SMO guidelines. 
• Perform other duties as assigned.  

Position Specifications
Education: Master’s degree from accredited university or college in human services, social or behavioral science required. License as a Mental Health Professional is required.
Experience: Three years of experience in the human services field required, preferably with different systems, including schools, behavioral health, child welfare, juvenile justice, health, and delinquency population.
Skills: Requires working knowledge of community-cased (and where applicable, delinquency case management), and juvenile court policies, procedures strongly preferred. Requires strong verbal and written communication. Effective interpersonal skills, ability to relate to wide diversity of people and situations, both internal and external, demonstrated good judgment. Strong knowledge of the technical and legal issues regarding case management of committed youth. Demonstrated skills in program development, organizational skills, time management, self-directed. The ability to learn the computer applications for Eckerd and SMO systems.
Physical Demands: Must be capable of extensive travel and driving. Must be free of communicable disease as defined by the state and to the extent that knowledge is attainable under the law. Must be able to work a flexible schedule that includes nights and weekends.
Other: Must meet state criminal background check requirements.
Travel: Must maintain an appropriate and valid state driver’s license. Must be able to meet requirements for Eckerd’s Auto Insurance and be able to drive for business purposes.
