The Medical Records Charge Capture Specialist enters all charges corresponding to care provided and procedures performed in assigned departments/service areas utilizing established coding and charging guidelines for accurate compliant charge capture and appropriate reimbursement. Ensures that medical record documentation supports these charges and that charges post to the patient's account. PART III: POSITION RESPONSIBILITIES (ESSENTIAL FUNCTIONS)
Accurately assigns appropriate charge codes for all procedures and services documented in the medical record.
Adds modifiers to chargemaster-driven CPT codes according to accepted coding rules and guidelines.
Applies correct infusion and injection hierarchy.
Carves out observation hours per established guidelines.Qualifications
Assists with efforts to increase practitioner awareness of documentation requirements to full capture patient severity and charges
Collaborates with source departments, HIM, PFS, and Revenue Integrity teams to speed revenue cycle and optimize corresponding revenue
Follows up on outstanding documentation on a daily basis, reporting these deficiencies to HIM management
Participates in assigned charge capture meetings
Prompts physicians and clinical team to complete essential medical record charting to document care delivered, support charges for these services, and maintain timely chart completion.
Verifies that charges post to the patient's corresponding account for timely and accurate billing
- High school diploma or equivalent required
- Associate's or Bachelor's degree in a health-related field highly recommended
- Medical terminology course
- Current and comprehensive understanding of CPT coding principles and modifier use essential
- Strong working knowledge of medical procedures and patient care items
- Prior health care charge capture experience highly preferredergency Department setting
License or Certification Requirements:
- Certified coding associate (CCA), Certified coding specialist (CSS), certified professional coder (CPC), registered health information technician (RHIT) or RHIT-eligible; and/or significant comparable experience in a similar acute care setting.
