Responsibilities

  • 20-25 hrs/weekly
  • Provide quality patient care based on an individualized plan of care
  • Record your patient’s health status and report per agency protocol
  • Provide companionship and personalized care while encouraging independence and self-care
  • Assist patients with activities of daily living, including but not limited to dressing, bathing, grooming, toileting, meal preparation, assistance with eating, etc… to help improve our patients’ quality of life while managing their care
  • Work collaboratively with a registered nurses and other clinicians

Qualifications

  • Valid driver’s license and insurance
  • Willing to drive to locations within Southlake, TX and surrounding areas
  • CPR certification
  • Ability to lift at least 50 pounds
  • Availability to work on weekends, evenings, and holidays
