Job Description

Mercy LIFE, a member of Trinity Health PACE, is seeking a Day Center Manager for its location in W. Springfield, MA. The Program of All-Inclusive Care for the Elderly (PACE) model is centered on the belief that it is better for the well-being of seniors with chronic care needs and their families to be served in the community whenever possible. PACE serves individuals who are age 55 or older, certified by their state to need nursing home care, able to live safely in the community at the time of enrollment and live in a PACE service area. While all PACE participants must be certified to need nursing home care to enroll in PACE, only about 7 percent of PACE participants nationally reside in a nursing home. If a PACE enrollee needs nursing home care, the PACE program pays for it and continues to coordinate the enrollee's care. We encourage fellowship and independence while serving seniors and their families with dignity and respect. Delivering all needed medical and supportive services, PACE is able to provide the entire continuum of care and services to seniors with chronic needs while maintaining their independence in their home for as long as possible.

Under the supervision of the Center Director, manages the daily, operations of the PACE Center. Responsibilities include the planning, coordination and implementation of all activities in the PACE Center. Works to assure that recreational activities are provided as scheduled. In conjunction with the Clinic Manager, assures that Day Center Workers complete assigned daily tasks.

General responsibilities will include:

Coordinates in-services, and training for Day Center workers and activity aides according to program needs and state and federal regulatory requirements.

Ensures that the daily assigned job responsibilities are completed.

Acts as a member of, and provides leadership for, the PACE Interdisciplinary Team; actively communicates relevant issues to the team, ensures all team members are informed and up-to-date, participates in the development of the Plan of Care, and monitors and articulates participant's progress and decline.

Maintains the PACE Center and its equipment as required by plant operations, and state and federal agencies. Monitors operations to ensure consistent compliance with established quality assurance standards.

In conjunction with the Center Director and Quality Improvement Director, maintains the stability and reputation of PACE by insuring that all activities and operations are performed in compliance with local, state, and federal contracts, regulations, laws, accreditation, protocol, licensing and certification requirements governing PACE operations. Fulfills all administrative requirements, such as development Day Center Worker schedule, oversight and completion of daily-required documentation and reports associated with State and Federal agencies.

Provides triage and wellness nursing care to participants on a regular basis. Administers routine medications as needed and assesses participant need for medical interventions. Teaches individual participants and groups regarding disease management and self-care.

Maintains responsibility for dementia programming throughout the Mercy LIFE program. Provides education to employees regarding dementia care in class settings as well as informally on an ongoing basis.

Monitors PACE operational compliance with OSHA regulations and agency policies and procedures. Protects privacy and maintains confidentiality of all company procedures, results, and information about, employees, participants, clients or families.

Provides administrative on-call coverage on a rotation basis.

Ensures coordinating and scheduling daily delivery of home delivered meals, DME, supplies and miscellaneous items, as requested by the Mercy LIFE team.

Maintains high standards of business and ethical conduct in accordance with applicable federal, state and local laws and regulations including fraud, abuse and waste. Avoids activities, which could result in perceived or actual conflicts of interest. Maintains required licenses and certifications.

Candidates will have:

  • Education - Associates Degree plus five (5) years related experience or Bachelors degree plus two (2) years related experience. Registered nurse preferred. Completion of Alzheimer's Association Training required within 90 days of employment.
  • Experience - Two (2) years' management experience, including one (1) year working with the frail or elderly required.
  • Must have a valid driver's license, proof of insurance and have means of transportation.
  • Must be able to safely perform the essential job functions with or without reasonable accommodation.
