Admissions/Intake Counselor (*LVN's encouraged to apply)

Job Summary 

The admissions counselor works directly with physicians, nurses, and other medical professionals around the bay area to screen candidates for admission, perform insurance verifications, perform intake assessments, communicate directly with patients and their loved ones upon arrival our facility. This is not your \u0022Typical Floor Nurse\u0022 Position. So if you are looking for a change of pace, or just need something out of the ordinary, this may be a great opportunity for you! Personnel in this position function under the direct supervision of the Admissions Director. 

Duties and Responsibilities

  • Demonstrate the ability to work well on a team.
  • Perform Intake Assessment on newly arriving patients.
  • Review clinical/medical information presented from community partners in order to verify appropriateness of patients for our facility.
  • Complete patient coverage verifications electronically and on the phone.
  • Complete paperwork legibly and accurately according to patient's current diagnoses.
  • Demonstrate proficiency in the DSM-V
  • Answer and screen all inquiry calls.  Follows-up on inquiry calls to schedule potential admissions.
  • Collects clinical data to facilitate admission process. 
  • Ensures that admission forms are signed correctly and are placed in the medical record.
  • Develop and maintain a positive work climate and support the overall mission of the facility.
  • Regularly communicate with other team members and interface in a positive, constructive, and helpful manner to promote cohesiveness, reduce conflict, and provide for resolution.
  • Respect the dignity and confidentiality of patients.
  • Adhere to San Jose Behavioral Health's (SJBH) policy on dress code, including the wearing of the identification badge.
  • Participate in department in-service/training programs.
  • Follow through with special assignments and responsibilities as assigned.
  • Performs other duties as assigned.


Knowledge and Skills

  • Communicate regularly and effectively with multidisciplinary treatment team and administrative team.
  • Develop and maintain a positive work climate and support the overall mission of SJBH
  • Respect the dignity and confidentiality of patients, serving as their advocate.  Adhere to HIPAA guidelines.
  • Appropriate documentation related to department and facility policy and procedure.
  • All paperwork is documented according to department and facility/policy and procedures.
  • Other documentation completed in accordance with federal, state, The Joint Commission regulations and third party payers/managed care organizations.
  • Receive patient, family member and guests in a professional and courteous manner.
  • Demonstrate the ability to understand and react effectively to the unique needs of the organization.
  • Crisis Intervention
Qualifications and Requirements
  • LVN STRONGLY preferred.
  • Bi-Lingual (Spanish) STRONGLY preferred.
  • OR
  • MSW degree or Licensed Clinical Social Worker in the State of California.
  • Or
  • Bachelor’s degree with experience working in a direct patient care environment.
  • OR 
  • MSW degree or Licensed Clinical Social Worker in the State of California.
  • 1-3 years experience in mental health treatment facility preferred.
  • Ability to communicate effectively.
  • Ability to use standard office equipment such as copier, fax machine and other equipment as required.
  • Ability to use computer systems and ancillary programs such as Microsoft Word, Excel, Outlook and other programs as required.
  • Ability to be well organized
  • Ability to be punctual
