A health services organization in New York is currently seeking a licensed Registered Nurse (RN) to join their staff in Rockland County as their new Hospice / Home Care Nurse. In this role, Hospice / Home Care Nurse (RN) will be responsible for providing optimum patient care through the evaluation and development of a custom plan of care.


The Hospice / Home Care Nurse (RN) will:
  • Participate in the orientation of newly appointed employees
  • Regularly assess and prioritizes workload to ensure optimum productivity and quality of care/services
  • Assess patient care needs and intervenes to ensure that needs are met in an appropriate and timely manner: provide symptom management and specialized nursing skills related to end of life care
  • Collaborate with the Clinical and Support staffs to effectively coordinate the daily care of the patient: inform the Physician and other personnel of changes in patients’ needs and outcome of interventions
  • Develop and maintain effective relationship with patients and families; contribute to patient and family counseling and education regarding disease process, self care techniques, prevention strategies, end of life care process and dealing with ethical issues
  • Contribute to patient and family counseling and education
  • Participate in QAPI program and hospice sponsored in-service training in compliance with professional, regulatory and agency standards
  • Participate in ongoing interdisciplinary comprehensive assessment
  • Continuously assess and evaluate the quality of service delivered in order to ensure the highest possible level of care and customer/patient satisfaction; evaluates the patient/family response to care and interventions
  • Document assessments, visits, all communications/conferences/IDT meeting, care plans and discharge summaries
  • Handle other duties, as needed
  • 2+ years of Medical-Surgical experience; 1+ year of recent Bedside Nursing experience
  • Associate's and/or Bachelor's Degree in Nursing
  • NYS Registered Nurse (RN) license
  • Some experience caring for terminally ill patients and a basic understanding and commitment to the hospice concept
  • Ability to assess the needs of terminally ill patients and their families and to plan care to meet these needs
  • Basic knowledge of medications, treatments and therapies for pain and symptom control
  • Solid assessment, clinical, and documentation skills
  • Patient oriented
  • Great interpersonal skills
  • Excellent communication skills (Written and verbal)
  • Strong attention to detail
  • Highly organized
Desired Skills:
  • Experience in a Home Care, Hospice and/or Oncology role/setting
