Assumes responsibility for the MDS, and how it relates to facility CMI, Quality Assurance and Interdisciplinary Care Plans. Is also coordinator for Care Plan meetings and the communication with members of the team.
The intent of this job description is to provide a summary of the major duties and responsibilities of this position and shall not be considered as a detailed description of all the work requirements that may be inherent in the position

1.         Interviews new residents and gathers data for the MDS.

2.         Instructs other RN’s and LPN’s re: MDS and care plans.

3.         Completes computer data entry for MDS as required by state regulations.

4.         Maintains current knowledge of MDS and CMI by attending seminars.

5.         Assesses resident’s total care and coordinates care with the interdisciplinary team to improve CMI.

6.         Functions as a staff nurse on alternating weekends or PRN basis.

7.         Evaluates the quality and documentation on progress records and resident care plans and completes documentation as necessary.

8.         Assists in daily care of residents and feedings as needed.  Supervises licensed practical nurse and/or certified nurses aide.

9.         Follows direction of NHA, shift RN and/or DON.

10.       If necessary will assist with ADLS, rounds, lifting/transporting of residents.

11.       Documentation as required on nurses notes, monthly summaries, MDS and care plans.  Revises care plans as needed.

12.       Assists in teaching residents/staff as directed,

13.       Attends AM report.

14.       Reports pertinent information to appropriate persons.

15.       Maintains familiarity in use of mechanical lifts, pumps and other technical equipment.

16.       Participates in safety awareness, fire prevention and infection control practices.

17.       Assess total care of residents while making daily resident rounds.

18.       Actively seeks to involve residents/families/social service and all other disciplines in establishing short and long term goals of Rx.

19.       Identify potential problems and expected outcomes, and reports them to DON as necessary.

20.       Understands the legal and moral issues involved with nursing actions.

21.       Assists with orientation of new personnel.

22.       Seeks and accepts guidance of own actions.

ESSENTIAL FUNCTIONS:

Maintains confidentiality of all materials handled within the Network/ Entity as well as the proper release of information. 

Complies with Network and departmental policies regarding issues of employee, patient and environmental safety and follows appropriate reporting requirements. 

Demonstrates/models the Network’s Service Excellence Standards of Performance in interactions with all customers (internal and external).

Demonstrates Performance Improvement in the following areas as appropriate: Clinical Care/Outcomes, Customer/Service Improvement, Operational System/Process, and Safety.

Demonstrates financial responsibility and accountability through the effective and efficient use of resources in daily procedures, processes and practices.

Complies with Network and departmental policies regarding attendance and dress code. 

Demonstrates competency in the assessment, range of treatment, knowledge of growth and development and communication appropriate to the age of the patient treated.

OTHER FUNCTIONS:

Other related duties as assigned. 

ADDITIONAL REQUIREMENTS:

Must be capable of coping with peers, residents and changed deadlines.

            Must be able to withstand high stress situations and meet demanding deadlines.

            Attend continuing educational program.

QUALIFICATIONS)

(MINIMUM)

EDUCATION:

            Registered Nurse with a current Pennsylvania license.

            CPR certified.

TRAINING AND EXPERIENCE:

Previous experience in geriatrics working as an MDS coordinator with data entry knowledge preferred.

QA experience and knowledge of CMI preferred.
