We are seeking a Registered Nurse / RN with home health experience to join a well-known healthcare organization as a RN Home Health Case Manager

Daily Responsibilities:
- Completes an initial assessment of client and family to determine home care needs
- Initiates the plan of care and makes necessary revisions as client status and needs change
- Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the client and the family in the planning process
- He/she will have the autonomy to create their own schedule
Requirements:
- Must be a licensed RN in the state of OH
- Minimum of 2+ years of clinical experience
- Must have 1+ year(s) of home health and OASIS experience
Hours for this Position:
- Monday-Friday, 8AM-5PM (No nights, weekends, or holidays)
- Ability to make own schedule throughout the week
