Job Description

The positions sought are for Care Managers to work in home-based settings, helping seniors to navigate health care services with the goal of assisting them to remain at home when faced with medical and functional challenges.  These are case-building positions with pay per visit and mileage/training compensated.  Care Management is ideal for individuals wanting a secondary income (ie:postsecondary education students, retired health care workers, stay at home parents, or anyone wanting a flexible/independent part time job). There is no direct care involved.


  Job functions will include, but are not limited to:   


o  In-home visits


o  Hospital/facility visits when authorized


o  Facilitating conference calls between patients, physicians and the Care Manager as

    needed to clarify treatment plans, medication regimens or other urgent issues


o  Reconciling prescribed medication 


o  Assessing patients’ environment, functional, psycho-social status and financial well-being


o  Identifying and developing action plans to empower patients to remain and thrive at home


o  Connecting patients and their families with approved resources, services and professional

    intervention within their community to help them address medical,

    legal, housing, insurance and financial issues


o  Providing patients and their family/caregivers with educational material and coaching to

     empower them to manage their health and well-being


o  Completing required paperwork and documentation within the timeframe required


o  Collaboration with other disciplines and participation on interdisciplinary rounds


o  Maintaining professional care management responsibilities by setting appropriate boundaries,

    complying with job expectations and productivity standards, and seeking managerial

    consult as needed


o  Strategic and analytic thinking to address gaps in care                                                                                              



Job Requirements

Required experience: 

o  Complex Care Management/Case Management/Managed Care experience


o  Community based or In-Home visit experience


o  Experience working with either Geriatric, chronically ill or functionally challenged populations


o  Computer skills - Ability to use a variety of electronic information applications/software programs, electronic medical records experience 


o  Intermediate to Advanced computer skills and proficiency with Microsoft Word, Outlook, and Excel, excellent keyboard and web navigation skills 


o  Access to a secure computer in order to accommodate data entry within required timelines 


o  High speed internet connectivity (DSL or cable modem; recommended speed is 10Mx1M for optimal performance) 


o  Additional requirements:  Valid driver’s license, car insurance, and access to an automobile for home visits to member is required
