Join a team of competent and caring professionals as a Nurse Care Manager at Behavioral Health Network's One Care program in Westfield, MA! Through their organized nature and attention to detail, Nurse Care Managers use their problem-solving skills to identify the needs of individuals and discover solutions for the healthcare team to meet those needs. The Nurse Care Manager uses their broad knowledge of sound clinical theories and principles to provide high quality clinical care management services and consultation.

The Nurse Care Manager is team-based, making sure that all appropriate members in the Integrated Care Team are included in decision-making when they implement and review comprehensive, person-centered Integrated Care Plans. They also ensure positive health outcomes and prudent use of resources by monitoring the effectiveness of the Integrated Care Plans and making adjustments as needed. Collaborating and consulting with internal and external service providers, the Nurse Care Manager makes sure to meet the needs of the individuals we serve timely and efficiently while maintaining direct service productivity.

Nurse Care Managers will be able to increase their knowledge of behavioral health conditions and how these conditions co-exist with chronic health conditions. They will learn how to work collaboratively on multidisciplinary teams and build skills in team consultation, sharing their medical expertise with other behavioral health team members.

Through the coordination and integration of care, One Care assists adults ages 21-64 with medical, behavioral health, or substance abuse concerns in navigating the health care system and improving their health outcomes. Behavioral Health Network is a growing non-profit community behavioral health agency that has been providing services to children, adults, families and communities in Western Massachusetts since 1938. BHN provides comprehensive, outcome-driven behavioral health care. We are dedicated to offering high quality, affordable and culturally appropriate care to people of all ages and income levels in our constituent communities.

Nurse Care Managers at Behavioral Health Network have the following qualities:

  • Knowledge of mental illness, the interface between mental health and medical conditions and systems, substance abuse, and care management.
  • Familiarity with internet, word processing and emails systems.
  • RN License (required).
  • Associate's Degree in Nursing (required).

Motor Vehicle Records: provides a report on an individual's driving history in the state requested. This search will be run when driving is an essential requirement of the position.

Substance Abuse Testing: positions embedded with these specific healthcare organizations are subject to a pre-employment substance abuse test, which includes the testing for the consumption of THC (Marijuana)
