Crisis Managment-Case Manager
Adecco is currently assisting a client in their search in hiring a Case Manager to work for one of the largest nonprofit organizations in MD & VA.
📍 Location: Maryland & Virginia
💰 Pay Rate: $30 - $32/hr (based on experience, education, and location)
📅 Schedule: Monday – Friday | 8:00 AM – 5:00 PM

Job Summary:
As a Case Manager, you will play a crucial role in providing comprehensive case management services, assessing client needs, and connecting them with vital resources. Your work will help individuals overcome barriers to housing, employment, education, and wellness while ensuring compliance with program standards.
Key Responsibilities:
✅ Assess client needs in critical areas, including housing, employment, education, financial stability, addiction treatment, and mental/physical health.
✅ Coordinate care with facility personnel and community partners to ensure clients receive essential services.
✅ Facilitate client intake processes and maintain organized, confidential client records in compliance with HIPAA regulations.
✅ Develop and update Individual Action Plans (IAPs) to support client progress.
✅ Conduct home visits to provide case management services and ensure client well-being.
✅ Lead life skills classes to empower clients with tools for independence.
✅ Maintain accurate data entry in HMIS and other internal/external systems.
✅ Prepare progress notes, discharge summaries, and reports as required.
✅ Facilitate support groups and assist in substance abuse counseling (if certified).
Qualifications:
✔️ Bachelor’s Degree Required (Social Work, Psychology, Counseling, or related field preferred).
✔️ 2+ years of experience in social services or a related field (preferred).
✔️ Experience working with homeless individuals and/or veterans is a plus.
✔️ Strong communication skills, professional demeanor, and the ability to work independently or within a team.
✔️ Certification in Substance Abuse Counseling (preferred).
✔️ Experience in facilitating support groups (preferred).
✔️ Valid driver’s license (preferred; must meet insurance requirements and complete driver training).

Pay Details: $28.00 to $30.00 per hour
