Provides direct and indirect patient care activities under the direction of an RN. Assists patients with activities or daily living, provides for personal care, comfort and assists in the maintenance of a safe and clean environment for patients.

Provides direct patient Care under the supervision of RN/LPN in a competent, personalized, and compassionate manner.
A. Successfully demonstrates proficiency of basic skills, equipment, and procedures, including unit specific skills.
B. Assists RN, LPN or physicians staff with examinations procedures and other processes related to patient care.
C. Reviews and completes assigned tasks initiated by the interdisciplinary care team.
D. Promptly communicates problems to the interdisciplinary team.
E. Assists with and accompanies patients during admission, transfer, and discharge processes .
F. Provides activities of daily living care; which may include feeding, bathing, pericare/foley care, oral/dental hygiene, combing hair, back care, assistance with dressing, changing bed linen, cleaning over-bed table and bedside stand, straightening room, and other general care as needed.
G. Clears overbed table prior to meal times.
H. Promptly passes trays and assists with positioning patient and preparing food items if necessary.
I. Fresh ice water provided to patient at 0630 and 1830 as needed.
J. Promptly notifies nurse when patient complains of pain.
K. Proves general patient care such as position patients, lifting and turning, assisting in use of bedpan or commode, and ambulating per treatment plan. Uses body mechanics and available resources to provide for patient and coworker safety.
L. Vital signs per department routine, or as ordered by physician, may include temperature, pulse, respiration, weight, blood pressure, pulse oximetry. Immediately reports VS or patient condition that is not within hospital or department defined parameters.
M. Accurate Intake and Output measurement.
N. Checks for reddened areas or skin breakdown and reports to RN or LPN.
O. Keeps patients clean, dry and bed and room tidy at all times. Protects modesty by covering and draping patients.
P. Assists with restocking isolation carts & nurse servers.
Q. Complies with Isolation Precautions utilizing appropriate PPE when caring for patients in isolation.
R. Provides post-mortem care and assists in transporting bodies to the morgue.
S. Promptly corrects and/or reports unsafe conditions to the RN or LPN.
T. Adheres to policies and procedures of the facility and individual department.
U. Performs capillary blood sampling (PCx) per hospital policy with accurracy
V. Complies with hospital P & P with specimen labeling.
Documents in a manner which is complete, accurate and legible in all designated areas: supportive of core measures & bundle compliance standards
A. Assists with data collection (height, weight, VS) when new patient is admitted to clinical area; orients patient to department routines and hospital room.
B. Documents in real time in electronic medical record; VS, weight, height, adl's and intake and output data per hospital policy.
C. SCD if ordered, documentation will reflect use which is supportive of DVT prevention
D. Ebicer if ordered, documentation will reflect use
E. ADL's details are to be documented anytime care is provided especially bathing/linen changes/pericare & oral cares
F. Activity levels such as up to BR/up bedside commode/waling in hall/up chair/turns are to be documented each time activity is completed
G. Ensure that Foley securement devices are being used
Provides age specific knowledge and skills where applicable. Works effectively with various cultures and ethnic groups.
Communication skills as evidenced by:
A. Communicates verbally and non-verbally in a concise, effective and appropriate manner to all hospital personnel, physicians, patients, and family/significant others:
*Reports off to oncoming CNA/PCT/SNI prior to leaving hospital. Hand-off patient reports are complete, concise, and accurate utilizing SBAR.
*Communications with dignity and respect to all personnel, physicians, patients and visitors.
*Checks mailbox, email and bulletin boards each shift.
*In the event of a coworker conflict or disagreement, request to speak with coworker in private and communications concerns assertively and directly. If resolution is not obtained, seeks assistance from Charge Nurse, Clinical Coordinator, House Supervisor or Clinical Director.
*Notifies Unit Secretary or Charge Nurse before leaving unit.
B. Displays a positive attitude.
C. Works as a team with other area coworkers, assisting other clinical areas as needed and/or requested. Proactively seeks opportunities to assists in care of all patients.
*Ensures patient needs are met and ensures supplies available for the oncoming shift.
*Demonstrates cooperation and support as a team member.
E. Encourages patient education that complements indisciplinary care plan:
F. Offers suggestions toward improvement of patient care processes.
G. Orients new CNA/PCT coworkers to department procedures.
F. Answers all call lights and telephone promptly and professionally. Is able to answer questions or direct caller to appropriate personnel.
G. Uses appropriate tools and resources for age, cultural or ethical needs.

Actively supports Performance improvement Initiatives by following hospital policies, procedures, and programs; provides ongoing peer education (i.e. Customer Satisfactions, Physician Satisfactions, Co-Worker Satisfaction, and Process Improvement):

*Is responsive to staffing needs of the unit.
*Assists in maintaining unit cleanliness.

A. Identification of safety, infection control, and work environment hazards with prompt resolution and/or reporting of problems.

Because you’re committed to excellence, you understand the importance of being properly prepared for your role at Mercy. That’s why you’ll bring to your role the right set of qualifications:
Education: High School graduate or equivalent or current High School Student who is 17 years of age or older. Current Certified Nursing Assistant or equivalent military training. Current Basic Cardiac Life Support certification within three (3) months of hire.
Certifications: Current Arkansas certification for nursing assistants or completed one clinical semester in an accredited RN program.
Other: Ability to read and communicate effectively in English. Basic computer knowledge.
Preferred Experience: One year nursing aide experience.
Preferred Other: Additional languages preferred.
