POSITION TITLE: Team Leader, Diversion Acute Stabilization
FTE: 100 %
POSITION REPORTS TO: DAS Program Director
DEPARTMENT: MH Residential Program
SUPERVISES: No one

PRIMARY PURPOSE OF DEPARTMENT:
The primary function of the Mental Health Residential Department is to provide adult individuals with quality care, support and training based on the Recovery Model, providing opportunities for community integration. This is done in accordance with all federal, state, county and payer requirements as well as the Agency’s Vision, Mission and Values. This employee acts in accordance with all TCV policies and procedures.

PRIMARY PURPOSE OF THIS POSITION:
The primary purpose of this position is to function as a team leader with a multi-disciplinary team, and community supports to provide training and awareness with the ultimate goal of self-sufficiency and independence for the consumers served. Assists in the training, supervision of the Resident Advisors and direct care staff to promote the program goals and efficiency in accordance with policies and procedures.

WORK SCHEDULE:
The normal workweek for this position is 37.5 hours each week. This position is in a 24/7 program, so employees must be flexible to assist with coverage when needed for all shifts, weekends, and holidays. Overtime and on-call duties are required.

DESCRIPTION OF ESSENTIAL RESPONSIBILITIES AND DUTIES:
1.Agency Conduct Duties
a.Duty to act and communicate in a professional manner, in accordance with the Agency’s Mission, Vision, and Values.
2.Leadership Duties
a.Assist supervisor/director in orienting and training new staff and providing input into staff evaluations. Monitors daily staff performance on goals, assignments and documentation.
b.Coordinate and delegate the daily procedures/functions of the program. Ensure staff are performing duties and working with individuals in accordance to policy and procedures utilizing positive recover interventions and support to meet the consumer needs.
c.Supervises and assists in the monitoring of physical site, including resident rooms in compliance with regulations ensuring that timely maintenance requests are completed.
d.Ensures accessibility to provide 24 hour on-call coverage/consultation for call-offs and emergencies as per supervisory directives.
e.Provide effective recovery focused intervention when residents engage in inappropriate behaviors.
f.Takes a lead role in various domains such as: crisis interventions and admissions/discharges.
g.Serves on Safety Committees and others as assigned
3.ISP and Documentation Duties
a.Works with the individual to create and implement Individualized Service Plan (ISP).
b.Works with the individual to create and implement Individualized Crisis Plan (ISP).
c.Complete all documentation within 24 hours. Documentation includes but is not limited to: ISP’s, ISP Reviews, ISP Addendums, progress notes, Functional Assessment, Discharge/Transfer Summary.
d.Maintain a quality record keeping system to ensure all consumer records are in compliance with policy and regulations. Ensure that quarterly chart audits are completed and documented. Time sheets must be completed daily, and any expense vouchers are completed monthly.
4.Education and Facilitation Duties
a.Facilitate milieu/activity groups and ensure a structure program is provided for each individual on a daily basis.
b.Provides de-escalation and crisis intervention as needed.
c.Takes the lead role in training new direct care staff on all orientation as assigned by Supervisor.
d.Orients and engages new admissions ensuring that the individual understand the program and can be successful in achieving their short term goals5.Medical Duties
a.Provide accurate medication monitoring which includes ensuing dietary needs of individuals with diabetes, etc., addressing consumer health related concerns as well as procedure monitoring such as medication counts and observing consumer intake of prescribed medication per policies.
b.Ensure all consumer medical requirements are up to date and assists consumer with scheduling and accompanies, if necessary, to medical appointments and follow up with doctor documentations and recommendations.
c.Must follow Blood Bourne Pathogens and handling of all medical waste (needles) according to policy
and procedures.
6.Policy and Procedural Duties
a.Participate in all required meetings as scheduled (i.e. staff meetings, AURD).
b.Relate incidents to the supervisor promptly and completing and faxing Incident Reports within 24 hours.
c.Ensure safety of all consumers by reporting all unsafe conditions to the appropriate personnel, conduct fire drills as scheduled, and keep medications/controlled substances counted and locked up.
d.Clarifying and coaching employees to ensure compliance with policies and procedures to ensure program and consumer needs meet expectation.
e.Ensure that consumer funds are handled properly and safely.
f.Following all guidelines for the purchasing of food, goods for operations ensuring all paperwork is in compliance with agency guidelines.
g.Communicate timely with members of the treatment team and service coordinators to ensure coordination of care to maximize that the needs of the consumer are addressed in a timely fashion.
h.Complete other duties as assigned.

OTHER POSITION CHARACTERISTICS:
ACCURACY REQUIRED IN WORK:
Accuracy in following policies and procedures especially medication and medical monitoring is essential in maintaining the health of the consumers. Accuracy in following ISPs will assure maximum functioning levels, and greater success of the consumers. Paperwork and documentation must be completed to facilitate communication among shift personnel, as well as for licensing. Awareness of consumer location and activity at all times.
CONSEQUENCE OF ERROR:
Lack of supervision of the residents, medication errors can cause relapses, decompensations and behaviors that require immediate and sometimes requiring emergency medical attention or even causing death. Lapses in physicals/TB testing can result in inability to work, or non-licensure of program. Failure to keep adequate and timely paperwork/written communication can also cause problems between staff, sub par care, confusion and loss of facility license. Failure to report a missing consumer in a timely manner can result in consumer harm and the expedition of community interventions to preserve the consumer's welfare.
VARIETY AND PURPOSE OF POSITION CONTACTS:
This individual will interact with consumer treatment team members e.g. Service Coordinators (CTTs, etc.), therapist, family or significant others to provide the maximum of communications and programming as well as consistency of service for the consumer's needs. Communication with the following people may be necessary: families for updates, and ongoing communication, medical personnel, Access drivers, pharmacists and various community supports. Contacts with administration are as needed in accordance with the Chain of Command Policy. PHYSICAL EFFORT:
Physical effort of this position will require walking, hearing, talking, writing, reading, typing, bending, stooping, balancing, pushing, pulling, climbing stairs, lifting of objects over 50 pounds,

WORK ENVIRONMENT AND CONDITIONS:
May be exposed to communicable diseases. Work is primarily in a residential setting with persons with mental illness, substance abuse, and physical disabilities. Can be stressful at time.

MACHINES, TOOLS, INSTRUMENTS, EQUIPMENT AND MATERIALS USED:
This staff member will be required to use copiers/fax/scan machines, computer for all documentation, email, timesheets, etc. Must operate cell phones. May be required to drive Agency or personal vehicles for Agency business and/or accompany consumers via other public transportation,

SAFETY FOR OTHERS:
This individual is required to use good judgment as many decisions are made which will impact the safety of individuals, staff, and consumers and could defuse a potentially serious situation. All unsafe conditions must be reported to a supervisor or a member of the safety committee.

KNOWLEDGE, SKILLS AND ABILITIES, TRAINING AND EXPERIENCE REQUIREMENTS:
KNOWLEDGE, SKILLS AND ABILITIES:
Basic understanding of Mental Illness, ability to communicate affectively and professionally, effective problem solving skills, ability to work well in team approach, ability to do basic math functions, perform routine household duties, to read and write in English. Ability to set appropriate therapeutic boundaries, and to follow all Agency rules, regulations and procedures.
REQUIRED MINIMUM TRAINING:
Bachelor’s Degree in human/social service fields
ADDITIONAL TRAINING EXPERIENCE:
Two years of experience in like work recommended. Residential experience highly recommended.

CONTINUED TRAINING, LICENSE OR CERTIFICATION REQUIRED BY AGENCY, STATUTE AND/OR
REGULATION:
*Complete new hire training with in first six months* Maintain CPR/First Aid Certification
*Maintain physicals and required tests * Minimum 12 hours training per year
* HIPAA, annually* Comprehensive Crisis Management annually
*Compliance Plan, annually*Fire Safety, annually
*Confidentiality, annually*Crisis Intervention annually
*Bloodborne Pathogens/Universal Precautions, annually*Medication Training Monitoring annually
*All federal and state Clearance per regulations
* Keep PPD screening and physical up to date and submit documentation of same. (PPD is required annually and a physical is required every other year.) Complete Hepatitis vaccination series within first month of employment or
sign a waiver.
