Pharmacist, Admixture Short Hour-On Call (Day Shift), Gaithersburg, MD

Kaiser Ashton, MD (Onsite) Full-Time



Oversees and participates in the preparation of sterile products and coordinates the efficient operation of IV admixture services, oncology services or allergen services.




Essential Responsibilities:



  • Prepares, procures, packages, stores and distributes sterile products and supplies for oncology, clinic, Home IV and antigen patients within established procedures and legal requirements. These therapies include, but are not limited to: antibiotics, antineoplastics, total parenteral nutrition, small and large volume parenterals, pain medications, sterile ophthalmic preparations and allergen extract preparations, where applicable. Documents responsibility by signing prescriptions, batch sheets and other forms.

  • Reviews physician orders and provides drug information and pharmacokinetic advice when needed. Functions in the chemotherapy, I.V. drugs and antigen distribution system for home, infusion center, SNF or medical center clinic use.

  • Monitors drug therapy, lab tests, confers with medical staff providers and suggests changes in drug therapy, drug utilization or therapy monitoring methods as appropriate. Documents all clinical interventions and quality problems using the approved methods.

  • Explains procedures of the I.V. program to a patient or family member and counsels on their medications. Distributes drugs, flushing and delivery system educational materials to the patient or family member.

  • Orders and maintains drug, supply, and equipment inventories for the delivery of the admixture and antigen service. Maintains work area to ensure storage and work areas are clean and organized for a safe work environment. Resolves drug, supply and equipment problems as needed.

  • Demonstrates professional competency in accordance with current sterile compounding standards and participates in ACPE accredited continuing education programs. Assists in the training and education of pharmacy technicians, students and residents.

  • Provides direction to the pharmacy staff in resolving professional and/or clinical problems, trains employees in new procedures and techniques, monitors department staff for errors and adherence to the established department procedures.

  • Demonstrates proficiency in operating automated systems for prescription order entry, patient profile review or retrieval of clinical or statistical data. Maintains proper records related to medication/supply charts, controlled prescription record keeping and other legal and department records and procedures. Demonstrated proficiency with the pumps used in oncology and Home Infusion, where applicable.

  • Participates in departmental quality improvement initiatives and contributes to team or departmental projects directed toward the achievement of decreased costs, improved service efficiency and patient outcomes.

  • Assures compliance to all regulatory rules and regulations and Kaiser Permanente's policies and procedures to include but not limited to: Drug Enforcement Administration (DEA), State Boards of Pharmacy, HIPAA, Principles of Responsibility, Proper use of Kaiser Permanente personal computers and applications.

  • Provides after hours pager coverage as required.

  • Performs special duties and projects as required.







Basic Qualifications:


Experience



  • Minimum One (1) year of home infusion or hospital admixture experience required.

  • Per the National Agreement, current KP Coalition employees have this experience requirement waived.



Education



  • BS in Pharmacy or Pharm D required.



License, Certification, Registration



  • Licensed to practice pharmacy in the assigned jurisdiction required.





Additional Requirements:



  • Experience with computerized pharmacy systems and IV manual and automated equipment.

  • Ability to communicate pleasantly and effectively with members and coworkers.

  • Excellent oral and written communication skills.

  • Knowledge of aseptic technique.

  • Knowledge of medical terminology, pharmaceutical dosage forms and nomenclature and calculations required.

  • Basic accounting and purchasing skills.

  • Problem solving skills.

  • Ability to perform under pressure.

  • Knowledge of the customer focused service requirements of the organization.





Preferred Qualifications:



  • Board certification in specialty or Home Infusion certification preferred.
