Patient Care Technician

Central Valley Specialty Hospital Sebastopol, CA (On Site) Full-Time

The Long Term Acute Care (LTCH) Division of Sonoma Specialty Hospital is designed to provide care of medically complex patients who have experienced catastrophic illness and/or multi-system failure and are in need of intense rehabilitation services and an extended stay in an acute care setting. These patients usually come directly from a Short Term Acute Care Hospital (STCH) and are in need of vent weaning and acute rehabilitation services for a longer period of time to recover and return to their optimal level of health. The average length of stay (ALOS) of these patients is a range of 25-35 days which is a longer than average stay at a Short Term Acute Care Hospital. Care for these types of patients is provided by RNs, Respiratory Therapists, Wound Care Nurses, Physical Therapists, Occupational Therapists, and Speech Therapists. Also a collaborative team of specialty physicians, psychologists, dieticians, and social workers will focus on an individualized goal for each patient to recover and reach their independence and optimum level of a missing piece in the continuum of care within the health care system. FULL-TIME, PART-TIME Positions: Job Summary: Patient care technicians (PCTs) are responsible for maintaining medical records, assisting with patient care needs, and performing clerical and CQI duties. PCT is responsible for collecting and reporting of information to the supervising RN and completion of all patient care related documentation. Ensures the comfort and safety of the patients Education, Training and Licenses Required: High School Diploma or G.E.D. certificate, or equivalent Current CPR certification Medical Assistant, EMT, CNA experience preferred Experience/Skills: Six (6) months of healthcare experience preferred Good Verbal and written communication skills for regular contact with employees, patients and patients families Positive interpersonal skills Basic math and computer skills Good organizational skills Knowledge of basic medical terminology/anatomy Ability to work with others in a flexible, cooperative manner. Requires moderate mental application and concentration to handle varying procedures and interruptions Essential Functions: The PCT performs support functions for the unit, including basic clerical functions, order entry, supply ordering and maintenance of a tidy patient care and desk area. Ensures safety for patients at risk for self-harm, injury. Immediately notifies RN of any abnormal/unusual findings of patient conditions, problems or data. Prepares and maintains patient care area/rooms and ensures they are clean and orderly. Removes and stores extra equipment in a timely manner. Enters work orders for needed equipment repairs and ensures follow up. Manages patient supplies in a fiscally responsible manner. Ensures storage rooms are neat and orderly and meet regulatory environmental standards. Maintains safe environment by following DNV safety goals. Performs proactive rounding as directed and adheres to all hospital and departmental policies, procedures and regulatory requirements. Participates in all fall interventions and unit specific safety practices and goals. Customer Service: Answers phone calls, greets and directs all patients and visitors in a timely, friendly and courteous manner. Receives positive feedback from co-workers and patients. Supports unit goals to improve the patient experience. Assists with set up and/or feedings of patients when appropriate. Maintains supply par levels in department. Performs basic order entry, data entry and retrieval, and patient charges. Maintains break logs and communicates with all clinical staff prior to breaks and lunches to ensure continuity of care. Equipment Knowledge Required: Use of multi-line phone system Use of computer, knowledge of Microsoft office suite preferred. Competency with the electronic documentation system must be obtained within 30 days of hire with full proficiency by 90 days of hire.
