Position Overview
We are currently seeking a Licensed Clinical Social Worker, Licensed Marriage & Family Therapist, or Licensed Clinical Professional Counselor for the NorthShore Medical Group Primary Care Department located in Gurnee, IL. This is a full-time position and is benefits-eligible. Daytime and early evening hours, Monday through Friday.
What you will need:
Education: Master's degree from an accredited education program required
License: LCSW, LMFT, or LCPC license required
Experience: Two years' post-graduate experience preferred, including at least one full year of direct patient care/psychotherapy; healthcare and/or mental health experience preferred; expertise in CBT, DBT, ACT preferred; Bilingual (English/Spanish) preferred
Responsibilities
In this role, you will:
* Conduct initial assessments of new Adult patients, develop appropriate treatment plans, communicate care issues, and provide short-term psychotherapy services, in a Primary Care office and from home
* Regularly meet with, and collaborate with, Family Medicine Physicians, Primary Care Physicians, Psychiatrists, LCSWs, LCPCs, and LMFTs, and other Behavioral Health Care Managers, to ensure patient behavioral health treatment plans are well-coordinated
* Discharge planning including but not limited to education on community resources and referrals while collaborating with the health care team
* Document and bill for psychotherapy and collaborative care services on a daily basis in a Collaborative Care Model patient registry and in the NorthShore electronic medical record system (EMR)
* Review clinical questionnaires, progress notes, and other behavioral health clinical intake information, to identify appropriate level of care for patients in a Primary Care practice
